肝动脉栓塞化疗联合中药治疗原发性肝癌临床观察

目的:观察肝动脉栓塞化疗(TACE)联合中药治疗原发性肝癌的临床疗效。方法:将在本院接受TACE治疗的原发性肝癌患者83例,随机分为2组,对照组40例仅接受TACE,研究组43例在TACE的同时联合中药治疗,根据实体瘤疗效评价标准对所有患者进行疗效评价。结果:经2疗程TACE术后1月复查CT评价临床疗效,研究组完全缓解(CR)1例,部分缓解(PR)25例,稳定(SD)11例,进展(PD)6例;对照组CR 0例,PR 18例,SD 16例,PD 6例。中位随访时间为12.5月,研究组和对照组1年生存率分别为72.1%(31/43)和57.5%(23/40)。治疗过程中大多数毒性都是轻微、一过性的,随访过程中并未发现严重不良事件。在Ⅲ~IV度的毒性反应方面,研究组大多数毒性反应如恶心、呕吐、肝区疼痛、白细胞降低、血小板降低、贫血、肝功能损害的发生率较对照组显著降低,差异均有显著性意义(P<0.05)。结论:TACE联合中药治疗原发性肝癌能显著提高临床疗效,同时降低毒性反应的发生率及程度。

Clinical Observation of Transcatheter Hepatic Arterial Chemoembolization Combined with Chinese Herbal Medicine in Treating Primary Liver Cancer

Objective:To investigate the clinical effect of transcatheter hepatic arterial chemoembolization(TACE) combined with Chinese herbal medicine for the treatment of primary liver cancer.Methods:Eighty-three inpatients with primary liver cancer received TACE were randomized into two groups.Forty patients in the control group were received TACE alone,the other 43 patients in the treatment group received TACE combined with oral use of Chinese herbal medicine.The solid tumors criteria were used for the evaluation of the therapeutic effect.Results:One month after two periods of TACE,computer tomography was carried out to evaluate response effect.One case had complete response(CR),25 had partial response(PR),11 had stable disease(SD),and 6 had progressive disease(PD) in the treatment group.In the control group,no one had CR,11 had PR,16 had SD,and 6 had PD.After a median follow-up of 11.3 months,one-year survival rate was 72.1%(31/43) in the treatment group and was 57.5%(23/ 40) in the control group.During the follow-up periods,most of the toxicity was mild and transient,and no severe adverse events occurred.As for the grade Ⅲ～Ⅳ toxicity,the incidence of adverse reactions such as nausea,vomiting,hepatalgia,leukopenia,thrombocytopenia,anemia and hepatic impairmentwas lower in the treatment groupthan that in the control group(P < 0.05).Conclusion:TACE combined with Chinese herbal medicine can enhance the clinical effect of TACE for primary liver cancer,and can decrease the incidence of adverse reactions and their severity.
