流动人口肺结核患者跨区域管理综合干预措施成效分析

目的评价综合干预措施在流动人口肺结核患者跨区域管理中的作用和效果,为今后相关政策的制定提供科学依据。方法对比分析实施综合干预措施前(2007年10月至2009年9月)和实施综合干预措施后(2009年10月至2011年9月)转出患者的主动告知率、到位率、代管率和完成治疗率,并通过深入访谈方式了解患者未到位治疗原因和转入地结防机构拒绝代管的原因。结果综合干预措施实施后,提前告知率、到位率、代管率和完成治疗率均大幅提升,分别从干预前的54.0%、58.1%、27.0%和80.6%提高到干预后的82.6%、79.6%、68.4%和95.6%,差异有统计学意义(P<0.05)。结论流动人口肺结核患者跨区域管理综合干预措施成效显著,值得推广和借鉴。政府仍需对该项工作加强规范管理力度。

Management of patients with tuberculosis among the mobile population

Objective To evaluate the results of intervention policy on the tuberculosis patients among the mobile population,and to provide scientific basis for the future design of the related management policy.Methods Contrast and analyse the informing rate,the arrival rate,the adopting-subsequent-treatment rate and the treatment-completion rate of the patients who transferred into other districts before(1 st October,2007 to 30 th September,2009) and after(1 st October,2009 to 30 th September,2011) implementing the combined method of intervention,and investigate the reasons of failure to arrive and refusal to adopt subsequent therapy by in-depth interview.Results The informing rate,the arrival rate,the adopting-subsequent-treatment rate and the treatment-completion rate increased significantly(respectively from 54.0%,58.1%,27.0% and 80.6% to 82.6%,79.6%,68.4% and 95.6%) after the combined method of intervention was implemented(P <0.05).Conclusion The combined method of intervention has made a great progress in trans-regional management,and the method should be promoted.Government still needs to put more efforts into the regulation of the trans-regional management.
