高血压微量白蛋白尿与胰岛素抵抗的相关性

摘    要:目的探讨原发性高血压(EH)微量白蛋白尿(MAU)与胰岛素抵抗的关系。方法选择50例EH患者,采用放射免疫法测定早晨第一次尿中微量白蛋白,将其分为微量白蛋白尿(MAU)组及无微量白蛋白尿(NMAU)组。并对所有患者测定血清脂质各项指标、空腹血糖(FBG)、空腹血胰岛素(FIns),计算胰岛素敏感指数(ISI),进行统计分析。结果EH伴MAU组与EH伴NMAU组比较,血清三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、FIns显著增高(P<0.05,P<0.05,P<0.01)。而高密度脂蛋白胆固醇(HDL-C),ISI显著降低(P<0.05,P<0.01)。结论伴有MAU的EH患者常存在胰岛素抵抗及脂代谢紊乱。

Abstract:Objective To investigate the relationship between microalbuminuria( MAU ) and insulin resistance in patients with essential hypertension. Methods 50 patients with essential hypertension were included in this study. Radio immunoassay was employed to determine MAU in the first urine in the morning. Based on above result, the patients were divided into two groups : MAU group and non MAU ( NMAU ) group. The lipid profile, fasting plasma glucose,fasting plasma insulin were measured and the insulin sensitive index ( ISI ) was calculated at the same time. Results There were significant increase of TG, LDL-C, Fins, (P 〈 0.05, P 〈 0.05,P 〈 0. 01 ) and marked decrease of HDL -C, ISI in MAU group ( P 〈 0. 05, p 〈 0.01 ) compared to NMAU group. Conclusion MAU often coexists with insulin resistance and dyslipidosis in patients with essential hypertension.
