改良超滤连负压吸引在小儿体外循环中的应用

目的探讨改良超滤连负压吸引技术在小儿体外循环（CPB）心脏手术中对输血及术后出血的影响。方法30例接受CPB下心脏手术的先天性心脏病患儿,分为常规超滤组（CUF组）和改良超滤组（MUF组）。观察术中库血用量、血浆用量、红细胞比容的变化,并用SSPS/PC进行统计学处理。结果MUF组库血用量、血浆用量、显著低于CUF组（P〈0.01）,且滤出水量明显多于CUF组（P〈0.01）。结论在婴幼儿心脏手术中,改良超滤可有效排出体内水分,提高红细胞比容,明显减少输血及术后出血,是节约用血的重要手段之一。

Application of Modified Ultrafiltration Plus Suction Drainage in Infants with Extracorporeal Circulation

OBJECTIVE To investigate the effects of modified ultrafiltration plus suction drainage technique on extracorporeal circulation in infants. METHODS Total 30 patients with congenital heart diseases undergoing required cardiopulmonary bypass（CPB） were randomly divided into the modified ultrafiltration（MUF） group and the conventional uitrafiltration（CUF） group.The dosage of bank blood and plasma,and the change of HCT were observed during the operation.The results were analyzed by SSPS/PC software.RESULTS The dosage of bank blood and plasma were significantly less in the MUF group than the CUF group（P〈0.01）.The MUF group had a larger fluid removal compared with the CUF group（P〈0.01）.CONCLUSION MUF can reduce body fluid and raise HCT and reduce blood transfusion need,postoperative bleeding and bank blood requirement in infants undergoing cardiac surgery.
