单球囊扩张下腔静脉治疗布—加综合征

１９９１年４月至１９９５年８月１８例下腔静脉阻塞所致布－加综合征患者接受了单球囊扩张治疗，其中膜型１０例，段型８例，除１例段型穿刺激破膜失败外，余１７例均成功地扩张致１０－２０ｍｍ，ｇｕｆｑ１５．２ｍｍ，阻塞下方下腔静脉压力从平均２．８ｋＰａ下降至１．４ｋＰａ，随访１－５４月，除２例段型术后出现再狭窄外，余１５例均下腔静脉通畅，症状缓解。上下腔同时插管造影，阻塞段部位、长度显示清楚，同时上方之导管

Dilation of the inferior vena cava with single balloon catehter for the treatment of Budd-Chiari Syndrome

Eighteen patients with Budd -Chiari Syndrome(BCS) associated with obstruction of the inferior vena cava(IVC) were treated by dilation with single balloon catheter between April 1991 and Augst 1995. There were 10 membranous type and 8 segmental type. The obstructed IVCs were dilated successfully from 10 to 20mm(mean 15. 2mm) in diameter in 17 and unsuccessfully in 1 with segmental type. The caval pressure below the obstruction was decreased from 2. 8 KPa to 1. 4 KPa after dilation. During follow -up preiods of 1 - 54 month the canals of IVC were patent in 15 patients with symptoms released and stenosis in 2 with segmental type. Simutaneous inferior vena cavography above and below the obstruction can precisely reveal the location and length of the obstruction. The catheter above the obstruction may serve as a targer during puncture. So long as the direction of the puncture needle was controled correctly by stereoscopic fluroscopy and locating repeatly the puncture of the segmantal obstruction could be achieved . The femoral vein needn't be multiplly punctred when the larger balloon catheters were used. Because the waist of the balloon strided over the obstruction the dilatation was definite and complete. We conclude that the procedure was a more simple, safe and effective method for the treatment of BCS, not only for membraneous type, but also for segmental type.
