血清转铁蛋白受体对小儿缺铁性贫血的诊断评价

目的:探讨血清转铁蛋白受体(sTfR)在儿童缺铁性贫血(IDA)诊断中的意义。方法:6月~12岁缺铁性贫血儿童50例及正常儿童20例均进行血红蛋白(Hb)、血清转铁蛋白受体(sTfR)、血清铁蛋白(SF)、血清铁(SI)、总铁结合力(TIBC)测定,并将受测对象分为正常对照组(C组)、轻度IDA组(IDA1组)、中-重度IDA组(IDA2组),以1989年洛阳会议制定儿童IDA诊断标准为据,以不同浓度sTfR为临界点计算其诊断IDA的敏感度及特异度,以确定最佳的诊断临界点。结果:以sTfR浓度40nmol·L-1为缺铁性贫血的诊断标准时,其敏感度和特异度分别为90%和85%,为诊断本组IDA敏感度和特异度之和最大的临界点。结论:当sTfR为40nmol·L-1时诊断IDA准确度最高,即为IDA的最佳临界点。

Study on Clinical Significance of Serum Transferrin Receptor in the Diagnosis of Iron Deficiency Anemia

Objective: To observe the clinical significance of the serum transferri n receptor (sTfR) in the diagnosis of iron deficiency anemia (IDA) in children. Methods: 20 normal controls and 50 IDA patients aged 6 months t o 12 years were involved for the determination of sTfR, Hb and serum ferritin (S F). All the children were divided into three groups: the normal control group (C ), mild IDA group (IDA_1 ) and moderate to severe IDA group(IDA_2). Am ong the patients, the sensitivity and specificity was analyzed at the different critical points of the sTfR, to definite the best critical point for diagnosis. Results: When the critical point of the sTfR was 40 nmol·L~ -1, the sum of its sensitivity and specificity to diagnose the IDA was the lar gest, so it's the best critical point to diagnose the IDA. Conclusion: The best concentration to diagnose IDA of sTfR is 40 nmol·L~-1, a nd sTfR is closely related to the severity of IDA.
