颈内动脉海绵窦瘘血管内栓塞治疗的临床价值

目的　探讨颈内动脉海绵窦瘘的血管内栓塞治疗。方法　对 18例外伤性颈内动脉海绵窦瘘患者 ,全部经皮股动脉穿刺行全脑DSA检查确诊 ,用可脱球囊进行栓塞治疗。结果　 18例患者全部栓塞成功 ,其中 15例同时保持颈内动脉通畅 ,1例CCF瘘口较大 ,2例球囊进入瘘口困难 ,改行闭塞瘘口的同时栓塞同侧颈内动脉。术后除 1例患者眼睛失明未恢复外 ,其余病例临床症状及体征完全消失。结论　血管内栓塞技术是目前治疗CCF的最好方法 ,具有损伤小 ,安全性高 ,疗效可靠等特点

Embolization Treatment with Detachable Balloon for Carotid Cavernous Fistulas

Objective To explore endovascular embolization treatment for carotid cavernous fistulas ( CCF ). Methods Selective cerebral angiography was performed in 18 patients , and CCF was showed in all . Then all of them were treated with detachable balloon .Results CCF was successfully embolized with the carotid remaining patent in 15 cases . Both of the fistulous orifice and carotid were embolized in 3 cases , as the fistulous orifice was too large ( 1 case ) or the balloon catheter enter difficultly fistulous ( 2 cases).The symptom was disappeared and all the patients became free except for one patient who remained blind after the therapy.Conclusion Being less invasive ,more safe and effective , endovascular embolization with detachable balloon is the best way for treating CCF .
