环氧合酶-2检测在胃肠肿瘤中的应用

目的　探讨环氧合酶 -2 (COX -2 )检测在胃肠肿瘤中的应用价值。方法　采用ABC法免疫组化检测 2 0例正常胃黏膜、12例早期胃癌、2 8例进展期胃癌和 2 3例正常大肠黏膜、3 4例大肠腺瘤、43例结肠癌组织标本中COX-2的表达。结果　①正常胃黏膜、早期胃癌和进展期胃癌的阳性率分别为 5 %、5 8.3 %、64 .3 %。早期胃癌和进展期胃癌的阳性率明显高于正常胃黏膜 (P <0 .0 1)。②大肠腺瘤和结肠癌的阳性率分别为 86.7%和 71.4%,明显高于正常大肠黏膜 16.7%(P <0 .0 1) ,其中高、中、低分化腺癌之间的阳性表达率无显著性差异 (P >0 .0 5 ) ,但高分化腺癌的阳性表达积分明显高于中、低分化腺癌 (P <0 .0 1)。结论　胃癌、大肠癌及大肠腺瘤COX -2的表达明显增高 ,并且在结肠癌中COX -2的表达与组织学分型有一定的关系 ,COX -2表达的检测对胃肠肿瘤研究和防治有重要意义。

The application of testing COX-2 in gastrocolonic tumors

Objective To explore the difference of expression of cyclooxygenase-2 (COX-2) between gastrocolic tumors and its clinic appliance value.Methods ABC method was applied to detect COX-2 expression in 20 cases of normal gastric mucosa,12 cases of early phase gastric carcinoma,28 cases of progressive gastric carcinoma,12 cases of normal colonic mucosa,30 cases of colonic adenoma,42 cases of colonic carcinoma.Results 1.Positive rate of COX-2 expression was 5%.58.3%,and 64.3% respectively in normal gastric mucosa,early phase gastric carcinoma and progressive gastric carcinoma.The last two was higher apparently than that in normal gastric mucosa (P<0.01).2.positive rate of colonic adenoma and colonic carcinoma,was 86.7% and 71.4%,which was apparently higher than that in normal colonic carcinoma,16.7% (P<0.01).There's no difference of COX-2 among high,medium and low differentiation of adenocarcinoma.Conclusions COX-2 expressed higher in gastric carcinoma,colonic carcinoma and colonic adenoma,Its expression in colnic carcinoma was related with the histologic subtype.It's of much importance to detect COX-2's expression in the research of gastrocolonic tumors.
