超声引导经皮冷循环微波消融治疗子宫肌瘤的初步探讨

目的探讨冷循环微波消融治疗子宫肌瘤的临床应用价值。方法应用超声引导经皮穿刺,将冷循环微波天线置入子宫肌瘤病灶内进行消融治疗,术后每3-6个月复查一次,以彩色多普勒超声或/和超声造影检查,并结合临床症状等综合判断和评价疗效。结果 58例手术均顺利完成。术后随访6个月至2年,58例中病灶内彩色血流信号消失55例（94.83%）,病灶边缘见少量点状彩色血流信号3例（5.17%）。其中,术后同时做超声造影21例,病灶内强化消失21例（100%）。子宫肌瘤瘤体缩小47例（81.03%）,无明显变化11例（18.97%）。临床症状：减轻49例（84.48%）,无变化9例（15.52%）,无加重病例。结论冷循环微波消融治疗子宫肌瘤,有微创、安全、有效及保全子宫的优点,是一种较有价值的治疗方法。

Ultrasound-guided percutaneous cold circulation microwave ablation for the treatment of hysteromyoma

Objective To discuss the clinical practice value of the cold circulation microwave ablation for the treatment of hysteromyoma.Methods Appling the ultrasound-guided percutaneous puncture,a cold circulation microwave antenna was placed directly into the focus of the hysteromyoma for ablations.Patients should be reviewed every 3 to 6 months after treatment by color Doppler ultrasound or/and contrast-enhanced ultrasound,and asked about clinical symptoms to judge and evaluate the therapeutic effect.Results Operations was performed successfully in all cases of 58 patients.Following up 6 months to 2 years,there were 55 cases（94.83%）of the color flow signals within the lesions disappeared in 58 cases,and there were 3 case（5.17%）of a small amount of punctate color Doppler signal being seen on the edge of the lesions.Enhancement within the lesions disappeared in all of the 21 cases（100%）which were implemented contrast-enhanced ultrasound at the same time after the treatment.There are 47 cases（81.03%）of the shrinking lesions,11 cases（18.97%）of no obvious change.The clinical symptoms eased in 49 cases（84.48%）,didn＇t change in 9 cases（15.52%）,and no case exacerbated.Conclusion The treatment of the hysteromyoma by ul-trasound-guided percutaneous cold circulation microwave ablation has the advantages of minimal invasion,safety,ef-fective,and preservation of the uterus,and is considered as a valuable treatment.
