布地奈德雾化吸入对全身麻醉气管插管术后咽喉部并发症的缓解作用

目的观察布地奈德雾化吸入对改善全身麻醉气管插管患者术后咽喉部不适的疗效。方法 110例全身麻醉下行气管插管的甲状腺良性肿瘤患者随机分为A组（n=40,予以布地奈德2 mg术前1 h、术后6、24和48 h雾化吸入）、B组（n=40,予以布地奈德2 mg术后6、24和48 h雾化吸入）和对照组（n=30,术后同时段予以生理盐水雾化吸入）。观察各组患者术后咽喉部情况,评价不同方式雾化吸入对患者气管插管后咽喉部症状和体征的疗效。结果所有患者术后咽痛、咳嗽和声嘶症状的发生率分别为99%、13.6%和53.6%。A组术后各时段的咽痛症状及术后1、6和24 h声嘶症状评分优于B组（P〈0.05）;B组与对照组术后48 h的咽痛和声嘶症状评分差异有统计学意义（P〈0.05）,其他各时段差异均无统计学意义（P〉0.05）;三组咳嗽症状评分差异无统计学意义（P〉0.05）。A组术后1、6和24 h咽喉部黏膜反应评分优于B组和对照组,B组术后6、24和48 h评分优于对照组（P〈0.05）。结论布地奈德雾化吸入能有效减轻全身麻醉气管插管引起的咽喉部损伤,术前给药能起到一定的预防作用。
