低位切开高位扩创引流治疗高位复杂性括约肌间肛瘘的临床观察

目的探讨扩创引流法治疗高位复杂性括约肌间肛瘘的临床疗效。方法采用低位切开、高位扩创引流治疗31例(治疗组),低位切开、高位挂线引流治疗31例(对照组)。比较两组临床疗效、治愈时间和肛门功能等。结果两组临床疗效无显著性差异(P>0.05);两组治愈时间、齿线上肛管形态、肛门括约肌功能比较有非常显著性差异(P<0.01,P<0.05),即治疗组的改善优于对照组。结论低位切开、高位扩创引流治疗高位复杂性括约肌间肛瘘疗效可靠,并能保持齿线上肛管形态和肛门括约肌功能,缩短创面愈合时间。

Clinical Observation of Low Incision and High Opening-Drainage Therapy for High-Complex Intersphincteric Analrectal Fistula

Objective To evaluate the efficacy of low incision and high opening-drainage therapy for high-complex intersphincteric analrectal fistula.Methods Sixty-two subjects were randomly divided into two groups:treatment group in which 31 cases were treated with low incision and high opening-drainage therapy and control group in which 31 cases were treated with low incision and high-seton drainage.The clinical efficacy,healing time of analrectal fistula and the function of anal sphincter were observed in the two groups after operation.Results The curative rates were of no significant difference between two groups(P > 0.05) .The healing time,the shape of superior analrectal area and the function of anal sphincter improved better in treatment group than in control group(P < 0.01,P < 0.05) .Conclusion Low incision and high opening-drainage therapy is quite effective in treating high-complex intersphincteric analrectal fistula,with shorter treatment course,and better anal shape and sphincter function.
