肺硬化性血管瘤CT影像分析

目的探讨肺硬化性血管瘤(PSH)的CT特点,提高其术前诊断的准确性。方法回顾性分析22例经手术病理证实的PSH的CT特征。所有22例均行胸部CT平扫与动态增强扫描。结果病灶直径为1.5～4.8 cm,平均3.2 cm;有4例位于肺门区,18例位于肺外围(81.82%);20例病灶(90.9%)边缘光整、未见明显分叶征,2例病灶有浅分叶;16例病灶平扫病灶密度均匀,6例病灶密度不均匀,4例病灶(18.2%)有钙化;16例病灶增强后呈比较明显的均匀或不均匀强化,CT净增值44～58 Hu,6例病灶呈轻度均匀强化,CT值净增10～15 Hu。8例病灶(36.3%)可见周围晕征,其中2例伴有空气新月征;5例可见贴边血管征。结论 PSH除具备一般肺内良性肿瘤的特点外,明显均匀或不均匀强化,周围晕征伴空气新月征是其相对特征性改变。

The analysis on CT imaging features of pulmonary sclerosing hemangioma

Objective To discuss the imaging findings of pulmonary sclerosing hemanagioma(PSH),so that to improve the accurate rate of its diagnosis before surgery.Methods The CT findings of PSH confirmed by pathology in 22 cases were analyzed retrospectively at PLA No.150 hospital.All the cases underwent MSCT prencontrast scan and enhanced scan.Results Diameters of these lesions ranged from 1.5 to 4.8 cm,and the average diameter was 2.8cm;4 lesions were located in hilum of lung,18 lesions(81.82%) were located in peripheral lung tissue.The lesions were well defined and non-lobulated in 18 cases(90.9%),2 had slightly lobulated margins.The density of the lesions were hemogeneous in 16 cases and inhomogeneous in 6 cases(including 4 lesions with punctuate caloification);16 lesions were obvious homogeneous or heterogeneous enhancement after contrast administration,the absolute enhancement ranges from 44 to 58HU;6 lesions were slight homogeneous enhancement after contrast administration,the abosolute enhancement ranges from 10 to 15 HU.The areola in the surronding lung parenchyma were noted in 8 cases,2 lesions were accompanied with air-trapping zone,5 cases had the sing of vessels at their peiphery.Conclusion On the basis of general benign tumor features,obvious homogeneous or heterogeneous enhancement and the areola in the surronding lung parenchyma accompanied with air-trapping zone are the ralative characteristic presene of PSH.
