特发性肺间质纤维化死亡24例临床分析

目的 :探讨特发性肺间质纤维化 (IPF)的临床特征、治疗及预后。方法 :应用回顾性调查方法对 99例IPF患者中 2 4例死亡患者临床特点、血气分析、肺功能特点进行分析。结果 :2 4例死亡患者缓解组年龄显著偏大、肺功能损害更严重。 2 4例均合并肺部感染 ,其中 8例死于冠心病、2例死于猝死、1例死于肺栓塞、1例死于气胸 (纵隔气肿 )。结论 :虽然肺功能损害严重提示预后不良 ,但IPF患者死亡原因大部分并非死于IPF本身 ,而是死于其合并症如感染 ,尤其是长期应用激素合并二重感染者 ,以及合并心脏病、肺栓塞、气胸等

Clinical analysis of 24 cases of death In Idiopathic Pulmonary Fibrosis

Objectives:To investigate the clinical characteristic?treatment and prognosis of idiopathic pulmonary fibrosis (IPF). Methods: The clinical characteristic?blood-gases analysis and pulmonary function were analysed retrospectively in 24 death cases of 99 cases patients with IPF.Results: The age of 24 death cases were more older than the alleviative group and the pulmonary function injury were also more seriously.All of the 24 death cases were complicated with infection.Of them,Eight cases were die from coronary heart disease,Two cases die from sudden death,One case die from pulmonary thromboembolism,One case die from pneumothorax.Conclusion: Although it was suggested that poor prognosis were due to serious pulmonary function injury.But most of the death causes of the patients with IPF were complication,such as,infection,especially secondary infection due to using cortisone,and complicated with heart disease,pulmonary thromboembolism,pneumothorax etc,rather than IPF itself.
