肺癌患者与吸烟及非吸烟者支气管肺泡灌洗液T淋巴细胞亚群的比较

摘    要:作者对30例肺癌患者,26例吸烟者及25例非吸烟者的外周血和支气管肺泡灌洗液(BALF)中的T淋巴细胞亚群(T-LS)进行了测定.结果显示:外周血T-LS变化不大;BALF中CD_3%三组间无明显差异,但CD_4%、CD_4/CD_3比值肺癌患者<吸烟者<非吸烟者,而CD_3%则肺癌患者>吸烟者>非吸烟者.提示肺癌患者与吸烟者肺局部细胞免疫功能均有减退,其程度以肺癌患者为重。吸烟所致的肺局部T-LS的改变可诱发肺癌,而肺癌发展过程中又可加重免疫抑制,促进肿瘤生长,两者互为因果。

Abstract:The measurements ofT-lym-phocyte subsets (as expressed by, CD_3 CD_4and CD_?) in the peripheral blood and bron-chial alveolar lavage fluid (BALF) on 30cases of lung cancer, 26 cases of smokerand 25 cases of nonsmoker heve beendone. The results showed that there exist-ed no differences in the peripheral bloodof the above said subsets of lymphocyte. InBALF the percentage of CD_3 in all thelymphocytes did not show any significantdifference among the three tested groups,either (P>0.05). But the percentage ofCD_4. and the ratio of CD_4/CD_5 manifestedthe following peculiarity: patients withlung cancerssmokers nonsmokers(P<0.01). It was suggested that thecellular immunity as shown in ?he peri-pheral blood did not correspond with thefindings in the BALF. The cellular immu-nity of the lungs was decreased both inpatients with lung cancer and smokers,but more severely in the group of lungcancer patients. Smokers who have had changes ofabove said subsets of lymphocytes in thelungs may develop lung cancer. As thelung cancer advances, it may suppressbody immunity and in turn enhance thecancer growth. Both the cause and theeffect are interrelated. Therefore, the pro-vocative agents for cellular immunitymay act as one of the adjunctive therapiesfor patients with lung cancer. Giving upsmoking should play an important role inrecovering or promoting the immunity ofthe lungs and decreasing the incidence aswell as improving the prognosis in patientswith lung cancer.
