腮腺Warthin's瘤的CT表现

目的探讨腮腺Warthin＇s瘤的CT表现。方法收集经手术及病理证实的腮腺Warthin＇s瘤25例,分析其CT表现。结果 25例共51个病灶,单侧单发13例,单侧多发4例,双侧单发3例,双侧多发5例;在轴位图像上,14个病灶最大径自前内指向后外。25个病灶见贴边血管征,其中8个病灶见贴边血管浅分叶征;28个病灶呈椭圆形,23个呈类圆形;15个病灶密度均匀,36个密度不均匀;18个病灶可见囊变,其中13个呈囊实性改变;50个病灶边缘清楚,1个边缘不清。20例（40个病灶）接受双期增强扫描,动脉期病灶均明显强化,静脉期强化减退。结论贴边血管征及贴边血管浅分叶征是腮腺Warthin＇s瘤的特征性CT表现;病灶位于腮腺后下极、多灶性或双侧发病常提示本病。

CT manifestations of Warthin's tumor of the parotid gland

Objective To investigate the CT features of Warthin's tumor of the parotid gland. Methods CT findings of 25 patients with Warthin's tumor of parotid gland proved with surgery and pathology were retrospectively reviewed. Results Fifty-one lesions were found in 25 patients, including unilateral and isolated lesions in 13 patients, unilateral and multiple lesions in 4, isolated and bilateral lesions in 3, as well as multiple and bilateral lesions in 5 patients. The maximum diameters of 14 lesions were from anteriomedialis to posterolateral in axial CT images. There were 25 lesions with vaso-welt sign, 8 of which with vaso-welt superficial lobular appearance sign. The shape of lesions were oval (n=28) or round (n=23), well-circumscribed (n=50) or with unclear margin (n=1), homogeneous (n=15) or inhomogeneous (n=36) in density. Thirteen lesions showed cysti-solid changes in all of 18 lesions with cystic formation. Forty lesions in 20 patients enhanced obviously in arterial phase, but the enhancement decreased in venous phase. Conclusion Vaso-welt sign and vaso-welt lobular appearance sign are the CT features of Warthin's tumor. Bilateral or multiple lesions located in the posterior and inferior quadrant of the parotid gland suggest for Warthin's tumor.
