深化孕产妇管理 预防巨细胞病毒宫内感染

目的 :探讨深化孕产妇管理在预防巨细胞病毒 (CMV)宫内感染方面的意义。方法 :前瞻性随机对照研究。采用酶联免疫吸附试验 (EL ISA)检测有异常孕产史的育龄妇女 2 90 7例、孕妇 3 193例外周血中 CMV特异性抗体 Ig G和 Ig M,筛查出 CMV Ig M阳性育龄妇女 2 10例、孕妇 2 45例 ,观察中药对 Ig M转阴率以及孕妇的宫内传播率和妊娠结局的影响 ,观察育龄妇女治疗转阴后再次妊娠情况。结果 :中药治疗可使育龄妇女 CMV Ig M转阴率从自然状态下的 41.2 8% (45/10 9)提高到 77.2 3 %(78/10 1) (χ2 =2 7.91,P=0 .0 1)、孕妇从 41.94% (52 /12 4)提高到 75.2 1% (91/12 1) (χ2 =2 7.90 ,P=0 .0 1) ,孕妇宫内传播率从 41.46% (17/4 1)降低到 2 0 .83 % (10 /4 8) (χ2 =4.45,P=0 .0 3 ) ,流产等胎儿异常明显减少 (χ2 =5.43 ,P=0 .0 2 )。结论 :应用 EL ISA法 ,从孕前开始筛查 CMV Ig M阳性者并给予相应的治疗 ,应纳入孕产妇管理

Extend the range of gravida supervision to prevent cytomegalovirus intrauterine infection

Objective:To study the prevent ion effect of extending the range of gravida supervision on intrauterine infect ion of cytomegalovirus intrauterine infection Methods:The prospective randomized-controlled trials were done Total 2 907 cases of reproductive life women and 3193 cases of pregnant w omen with abnormal pregnancy history were studied Firstly screening the specifi c antibody IgG and IgM of cytomegalovirus in women's plasma was detected using E nzyme-linked immunoadsorbent assay (ELISA) 101 cases reproductive life women w hose cytomegalovirus IgM was positive took the traditional Chinese medicine—Jin yebaidu granule while other 109 cases did not have any medicine 121 cases grav icla whose cytomegalovirus IgM were positive also took the traditional Chinese m edicine while other 124 cases did not have any medicine Results: The traditional Chinese medicine can significantly rise the transnegative rate of cytomegalovirus IgM of reproductive life women from 4 1 28% (45/109)to 77 23% (78/101) (χ 2=27 91, P=0 01 ), and that of gravida from 41 94% (52/124) to 75 21% (91/121) (χ 2=27 90,P=-0 01 ) The traditional Chinese medicine can also sign ificantly lower the intra uterine infection rate from 41 46% (17/41) to 20 83% (10/48) (χ 2=4 45 ,P=0 03) and improve the fetal growth and development Conclusion:It should be one of the gravida supervision wo rk to screen cytomegalovims IgM positive women before pregnancy and to treat the m correspondingly
