棘突基底截骨椎管扩大成形术的临床应用

目的：通过棘突基底截骨椎管扩大成形术的临床应用，探讨治疗下腰椎疾病的新术式。方法：应用棘突基底截骨椎管扩大成形术治疗下腰椎疾病30例，术后按日本骨科学会的评定标准进行疗效部分和影像学分析．结果：平均随访2年2个月，平均改善率83．6％，优26例，良4例。结论：本术式操作简单，并发症少，对脊柱稳定性破坏少，可有效地预防硬膜和神经根的瘢痕粘连，中央管和神经根管的减压充分，是一种治疗下腰椎疾病的良好术式。

Clinical Application of Spinal Canal Plasty with Spinous Process Osteotomy

Objective:To evaluate the clinical outcomes of spinal canal plasty with spinous process osteotomy and explore the new surgical modality in the treatment of lower lumbar disorder.Methods:30 cases of lumbar disorder were treated with spinal canal plasty with spinous process osteotomy.The efficacy and imaging findings were analyzed based on criteria of Japanese orthopaedic academy.Results:Duration of follow-up was 2 years and 2 months.The average improvement rate was 83.6%,includins 23 cases with high efficacy and 4 good therapeutic effect.Conclusion:Spinal canal plasty with spinous process osteotomy is simple and effective for preventing adhesion between dura and nerve root and sufficiently reducing pressure in central canal and nerve root canal,with less complications and destruction.
