关节镜手术治疗膝关节半月板损伤的临床疗效

目的探讨关节镜手术治疗膝关节半月板损伤的临床疗效。方法68例膝关节半月板损伤患者,其中,退行性半月板撕裂34例(A组),新鲜半月板撕裂34例(B组)。两组患者均采用关节镜手术治疗。比较两组患者的手术指标(手术时间、术后下床时间、住院时间),术前、术后的膝关节功能评分,术后并发症发生情况。结果B组患者的手术时间(47.68±8.25)min、术后下床时间(3.95±0.62)d、住院时间(5.38±1.23)d均短于A组的(60.13±10.25)min、(5.58±0.72)d、(7.69±2.35)d,差异均具有统计学意义(P<0.05)。术前,两组患者的膝关节功能评分比较,差异无统计学意义(t=0.264,P>0.05);术后,两组患者的膝关节功能评分均高于本组术前,且B组患者的膝关节功能评分(93.48±4.42)分高于A组的(76.97±4.03)分,差异具有统计学意义(t=16.095,P<0.05)。B组术后并发症发生率5.88%低于A组的23.53%,差异有统计学意义(χ^2=4.221,P<0.05)。结论膝关节半月板损伤患者应用关节镜手术治疗是切实可行、有效的,能有效缩短患者手术时间,减少手术本身对机体的损伤,促进患者尽快出院,同时能在一定程度上改善膝关节功能,减少术后并发症的发生,利于患者提高生存质量,具有较高的应用意义。但新鲜半月板撕裂的手术效果优于退行性半月板撕裂。

Clinical effect of arthroscopic surgery on meniscus injury of knee joint

Objective To discuss the clinical efficacy'of arthroscopic surgery on meniscus injury of knee joint.Methods There were 68 patients with meniscus injury of knee joint,including 34 cases of degenerative meniscus tear(group A)and 34 cases of fresh meniscus tear(group B).Both groups underwent arthroscopic surgery.The surgical indicators(operation time,postoperative off-bed time and hospitalization time),knee function score before and after operation and occurrence of postoperative complications were compared between the two groups.Results The operation time(47.68±8.25)min,postoperative off-bed time(5.38±1.23)d and hospitalization.time(3.95±0.62)d of group B were all shorter than(60.13±10,25)min,(5.58±0.72)d and(7.69±2.35)d of group A,and the difference was statistically significant(P<0.05).Before operation,there was no statistically significant difference in knee function score between the two groups(t=0.264,P>0.05).After operation,the knee function score of the two groups were higher than those before operation,and knee function score(93.48±4.42)points of group B was higher than(76.97±4.03)points of group A,and the difference was statistically significant(t=16.095,P<0.05).The incidence of postoperative complications 5.88%of group B was lower than 23.53%of group A,and the difference was statistically significant(χ^2=4.221,P<0.05).Conclusion The application of arthroscopic surgery is feasible and effective in the treatment of meniscus injury of knee joint,which can effectively shorten the operation time of patients,reduce the injury of operation,promote the patients to discharge as soon as possible,at the same time,it can improve the knee joint function to a certain extent,reduce the occurrence of postoperative complications,and improve the quality of life of patients,which has high application significance.But the surgical effect of fresh meniscus tear is better than degenerative meniscus tear.
