地塞米松前房注射在老年性白内障小切口非超声乳化人工晶体植入术中的应用

摘    要:目的 探讨地塞米松前房注射在老年性白内障小切口非超声乳化人工晶体植入术中的应用.方法 80例(80眼)老年性白内障患者随机分成观察组和对照组.两组均行小切口非超声乳化囊外摘除人工晶体植入术.观察组植入后注入地塞米松针0.5 mg,对照组注入复方平衡盐溶液以维持前房.结果 观察组术后角膜水肿的发生率和前房纤维蛋白渗出发生率明显低于对照组.两组术后1d、7d、14 d眼压水平虽有一定波动,但与术前比较差异无统计学意义.随访半年,观察组患者术后后囊浑浊发生率明显低于对照组.结论 前房注射地塞米松治疗老年性白内障的应用可减轻角膜水肿和术后前房炎症反应,安全性较好,且可减少后囊浑浊发生率.

Abstract:Objective To discuss application of dexamethasone anterior chamber injected into small incision non-pha- coemulsification and intraocular lens implantation of senile cataract. Methods Eighty eases (eyes) of patients with se- nile eataxaet were divided into observation group and control group at random. The patients in two groups were both given small incision non-phacoemulsification cataract extraction and intraoeular lens implantation. The patients in ob- servation group were injected with 0.5 mg dexamethasone from temporal puncture after artificial lens were embedded. The patients in control group were only injected with compound balanced salt solution to maintain anterior chamber. Results The occurrence rate of corneal edema and postoperative anterior chamber fibrin exudation of patients in ob- servation group was much lower than that in control group. The intra-ocular tension levels of patients in two groups one day, 7 days and 14 days after the operation were fluctuated to some extent, but no statistical differences were ap- peared after comparison with that before operation. According to the half-a-year following-up. The occurrence rate of postoperative posterior capsule opaeification of patients in observation group was much lower than that in control group. Conclusion The application of dexamethasone anterior chamber can relieve the degree of corneal edema and inflamma- tory reaction of anterior chamber with no obvious influence on intra-oeular tension but high security, which can reduee the occurrence rate of posterior capsule opaeification.
