电视胸腔镜与开胸手术治疗重症肌无力的疗效比较

目的对电视胸腔镜与开胸手术治疗重症肌无力的疗效进行对比分析。方法本文研究对象为我院2010年1月~2013年1月间收治的重症肌无力患者30例,其中15例行胸腔镜手术,设立为观察组,其余15例患者行开胸手术,设立为对照组,比较两组的完全稳定缓解率、手术时间、出血量、住院时间及并发症情况。结果观察组与对照组的完全稳定缓解率差异不具有统计学意义(P>0.05)。观察组的手术时间虽然长于对照组,但术中出血量少、住院时间短,差异具有统计学意义(P<0.05)。两组患者术后均出现肺部感染,对照组5例(33.3%),观察组2例(13.3%),两组肺部感染发生率组间比较,差异具有统计学意义(χ2=4.237,P<0.05)。结论电视胸腔镜比开胸手术治疗重症肌无力具有比较显著的疗效优势,如明显改善患者的临床症状、出血量少及可以缩短住院时间,并发症少,值得广泛推广和应用。

The efficacy comparison of VATS and thoracotomy in treatment of myasthenia gravis

Objective To compare the efficacy of VATS and thoracotomy in treatment of myasthenia gravis. Methods 30 patients with myasthenia gravis in our hospital from January 2010 to January 2013 were treated, 15 routine thoracic surgery,the establishment of the observation group,the remaining 15 patients underwent thoracotomy, the establishment of for the control group,completely stable remission rate,operative time,blood loss,hospital stay and complications were compared between two groups. Results The complete stable remission rate of observation group had no significantly difference compared with the control group （P〉0.05）. Although the operation time of the observation group was longer than the control group, but less blood loss, shorter hospital stay, and the difference was statistically significant （P〈0.05）. Patients in both groups were pulmonary infection occurred,the control group had 5 patients （33.3%）,the observation group had 2 cases（13.3%）,the difference was statistically significant （x2=4.237 ,P〈0.05）. Conclusion The efficacy of VATS and thoracotomy in treatment of myasthenia gravis has significant advantages,such as significantly improved clinical symptoms ,less bleeding, can shorten the hospital stay ,fewer complications, should be popularized and applied.
