中心静脉导管闭式引流联合持续低负压吸引治疗老年性气胸的疗效观察、安全性评估及适应证分析

目的 探讨中心静脉导管闭式引流联合持续低负压吸引治疗老年性气胸的治疗效果、安全性及适应性.方法 将老年性气胸患者纳入研究对象,随机分为给予中心静脉导管闭式引流联合持续低负压吸引治疗的观察组和中心静脉导管闭式引流治疗的对照组,观察治疗相关指标、不良反应及不同疾病类型的治疗效果.结果 观察组治疗有效率（87.5％）明显高于对照组;COPD老年性气胸患者治疗有效率（42.86％）、复张时间（1.8±0.3）d、胸痛持续时间（1.3±0.2）d、拔管时间（4.8±0.9）d、卧床时间（5.6±1.1）d及住院总时间（8.1±1.5）d、NRS评分（1.3±0.5）、皮下水肿（5.0％）、发热（25.0％）、穿刺部位感染（2.5％）均明显低于对照组（P＜0.05）.结论 中心静脉导管闭式引流联合持续低负压吸引有助于改善治疗效果,降低不良反应,但对COPD老年气胸患者治疗效果较差.

Effectiveness,safety, adaptability of central venous catheter closed drainage combined with continuous low negative pressure drainage in treating geriatric pneumothorax

Objective To discuss effectiveness, safety, adaptability of central venous catheter closed drainage combined with continuous low negative pressure drainage in treating geriatric pneumothorax. Methods Geriatric pneumothorax patients were enrolled and randomly divided into observation group given catheter closed drainage combined with con- tinuous low negative pressure drainage and control group given catheter closed drainage. Therapeutic index, adverse re- actions, treatment effect of different types of illness were observed. Results Observation group efficient （87.5%） was significantly higher than the control group; COPD patients efficient （42.86%）,recovery time（ 1.8±0.3）d, chest pain du- ration（1.3±0.2）d,extubation time （4.8±0.9） d, bed time（5.6±11）d, hospitalization time（8.1±1.5）d, NRS score （1.3± 0.5）, subcutaneous edema （5%）, fever （25%）, puncture infection （2.5%） were significantly lower than the control group （P〈0.05）. Conclusion Catheter closed drainage combined with continuous low negative pressure drainage by cen- tral venous have positive value for geriatric pneumothorax can improve therapeutic effect,reduce adverse reactions, but COPD geriatric pneumothorax patients treatment effect is poorer.
