生长抑素和垂体后叶素对兔门静脉压和肠系膜上动脉压影响的研究

摘    要:背景:生长抑素(SST)和垂体后叶素是最常用的止血药物,但两者对内脏血管压力的量效关系以及对动静脉作用的差异尚不完全清楚。目的:研究SST和垂体后叶素对兔门静脉压和肠系膜上动脉压的影响。方法:56只新西兰大白兔随机分成7组,其中3组分别给予SST 168μg/h(S1)、84μg/h(S2)、42μg(S3),3组分别给予垂体后叶素4U/h(P1)、2U/h(P2)、1U/h(P3),其余8只动物给予0.9%NaCl溶液50ml/h作为对照组。记录用药前和用药1、5、10、15、20、30和60min后门静脉压和肠系膜上动脉压.结果:SST可明显降低门静脉压和肠系膜上动脉压(P〈0.05),但剂量与效应不成正比;垂体后叶素呈剂量依赖性地降低门静脉压以及升高肠系膜上动脉压(P〈0.05)。结论:SST和垂体后叶素对门静脉压和肠系膜上动脉压的作用机制存在差异,临床应根据实际情况选择SST或垂体后叶素来止血。

Abstract:Background:Somatostatin(SST) and pituitrin are commonly used hemostatic agents,however,their dose-effect relationship on visceral vessel pressure and differential roles on artery and vein are not yet clearly known.Aims:To investigate the effects of SST and pituitrin on portal vein pressure and superior mesenteric artery pressure in rabbits.Methods:Fifty-six New Zealand rabbits were randomly divided into seven groups.Rabbits in three groups were given SST 168 μg/h(S1),84 μg/h(S2) and 42 μg/h(S3),respectively.Rabbits in another three groups were given pituitrin 4 U/h(P1),2 U/h(P2),1 U/h(P3),respectively.The rest 8 rabbits were given 0.9% NaCl 50 ml/h as control group.Portal vein pressure and superior mesenteric artery pressure before and 1,5,10,15,20,30,60 minutes after intravenous administration of the agents were recorded.Results:Portal vein pressure and superior mesenteric artery pressure decreased significantly in SST groups(P<0.05),but was not directly proportional to the dose.In pituitrin groups,portal vein pressure decreased while superior mesenteric artery pressure increased dose-dependently(P<0.05).Conclusions:The roles of SST and pituitrin in portal vein pressure and superior mesenteric artery pressure are different.Therefore,SST and pituitrin for hemostasis should be selected according to the clinical conditions.
