噻托溴铵对稳定期慢性阻塞性肺疾病患者肺功能和气道炎症的影响

摘    要:目的分析噻托溴铵对稳定期慢性阻塞性肺疾病(COPD)患者的肺功能和气道炎症的影响情况。方法将76例COPD患者随机分为实验组与对照组,实验组患者在原有维持治疗方案的基础上使用噻托溴铵治疗.对照组患者仅维持原有治疗方案,对比分析两组患者治疗前后肺功能相关指标、生活质量问卷(SGRQ)、6分钟步行距离(6MWD)、诱导痰检测结果,并对以上指标进行相关性分析。结果治疗后,实验组患者IC、6MWD以及SGRQ优于对照组,差异有统计学意义(P〈0.05);实验组患者诱导痰中性粒细胞、巨噬细胞明显优于对照组,差异具有统计学意义(P〈0.05);SGRQ与IC、6MWD呈负相关,与诱导痰中性粒细胞呈正相关:两组患者不良反应对比差异不具有统计学意义(P〉0.05)。结论采用噻托溴铵治疗稳定期慢性阻塞性肺疾病患者,能够显著改善深吸气量IC与呼吸困难症状,而肺功能相关指标变化不明显,诱导痰中性粒细胞比例显著升高.可以作为评价呼吸困难症状的指标之一,值得进一步推广研究。

Study on the effect of pulmonary function and airway inflammation of tiotropium bromide in patients with stable chronic obstructive pulmonary disease

Abstract:Objective To investigate the effect of pulmonary function and airway inflammation of tiotropium bromide in patients with stable chronic obstructive pulmonary disease (COPD). Methods Seventy-six cases of COPD patients were randomly divided into experimental group and control group, patients in experimental group used thiophene based on the original maintenance treatment scheme of tiotropium bromide therapy, patients in control group only maintained the original treatment plan, comparative analysis relevant indicators of lung function, quality of life questionnaire (SGRQ) before and after treatment in the two groups, 6 minutes walking distance (6MWD), induced sputum test re- suits, and the above indexes by correlation analysis. Results After treatment, IC,6 MWDS and SGRQ of the experi- mental group patients were superior to the control group, the difference was statistically significant (P 〈 0.05), induced sputum neutrophils of experimental group and macrophages were better than control group, there were statistically sig- nificant difference (P 〈 0.05). SGRQ was negatively correlated with IC, 6MWD, and induced sputum neutrophil cells were positively related, two groups of patients with adverse reaction of contrast difference were not statistically significant (P 〈 0.05). Conclusion Using tiotropium bromide in the treatment of patients with stable chronic obstructive pulmonary disease, can significantly improve the inspiratory capacity IC and symptoms of dyspnea, and related indexes of pulmonary function did not change significantly, the proportion of sputum neutrophils induced increased significantly, can be used as one of indexes to evaluate the symptoms of dyspnea, worthy of further promotion research.
