腹腔镜结肠次全切除联合改良Duhamel术治疗重度功能性便秘的疗效观察

目的探讨腹腔镜结肠次全切除联合改良Duhamel术治疗重度功能性便秘的疗效。方法将病例随机分为实验组和对照组,实验组采用腹腔镜镜结肠次全切除联合改良Duhamel术进行治疗,对照组采用结肠次全切除术进行治疗。结果在近期疗效方面两组患者在术后肛门排气时间、住院时间以及术后出现诸如大便失禁、腹泻、腹痛及肠梗阻等不良并发症方面两组比较差异不显著;在肠道功能恢复方面和临床症状改善方面比较,实验组显著优于对照组。而随访结果显示:两组患者在临床症状改善方面较术后早期均有所下降,而在患者排便频率满意率以及生活质量满意率方面实验组显著高于对照组在便秘复发率方面实验组显著高于对照组。结论使用镜结肠次全切除联合改良Duhamel术治疗重度功能性便秘的疗效显著,值得临床推广使用。

The Laparoscopic Colon Subtotal Resection Combined with Modified Duhamel Surgery Treatment of Severe Functional Constipation Clinical Observation

Objective To study the laparoscopic colon after excision with improved the curative effect of treatment of Duhamel severe functional constipation.Methods The cases were randomly divided into experimental group and control group,experimental group using laparoscopic colon lens subtotal resection with modified Duhamel art therapy,the control group using the colon resection for treatment.Results The curative effect in two groups of patients in the near future in the anus exhaust time,postoperative length of hospital stay and postoperative such as incontinence,diarrhea,abdominal pain,intestinal obstruction and other adverse complications in two groups of comparisons difference was not significant;in terms of intestinal functional recovery compared to improvements in clinical symptoms,the experimental group was significantly better than the control group.And follow-up results showed that two groups of clinical symptoms improved in patients with early postoperative were decreased,and the frequency of defecation in patient’s satisfaction and patient satisfaction and quality of life in the experimental group is significantly higher than control group in constipation recurrence rate of experimental group is significantly higher than that in control group.Conclusion Using colon lens times total excision with improved treatment of Duhamel severe functional constipation curative effect is distinct and worthy use in clinical.
