老年心肌梗死后抑郁与心理社会因素的关系

目的 探讨老年急性心梗后抑郁与病前生活事件、社会支持以及应付方式的关系。方法 采用Hamilton抑郁量表、生活事件量表、应付方式问卷、社会支持评定量表等量表评定30例心梗后伴有明显抑郁症状的老年患者，并与30例不伴明显抑郁症状的心梗患者以及30名健康人进行对照。结果 负性事件刺激量和生活事件刺激总量方面，抑郁组高于非抑郁组，非抑郁组高于健康组，且具有显著性差异。抑郁组的寻求帮助因子得分低于健康对照组；抑郁组的退避因子得分高于非抑郁组和健康对照组评分。抑郁组的主观支持得分低于健康对照组。结论 近期负性生活事件刺激量高、不适当的应付方式和缺乏社会支持是影响老年心肌梗死后抑郁症状的重要心理社会因素。

The relationship between post-myocardial infarction depression and psychosocial factors in elderly

Objective To explore the relation between post-myocardial infarction depression and psychosocial factors by control design. Methods Hamilton Depression Scale (HAMD), Life Events Scale(LES), Coping Scale Questionnaire (CSQ), Social Support Rating Scale (SSRS) and et al. were examined in 30 post-myocardial infarction (MI) depression patients, and examined in 30 MI patients without obvious depression symptoms and 30 health control in elderly.Results The depressed group showed significantly higher than nondepressed group and the latter showed significantly higher than health group (P<0.01)in negative stimulant intensity and total stimulant intensity. Patients group showed significantly lower than health group in looking for help scores while higher than disease control and health control group in retreat scores. The patients group was significantly lower than health group in subject support and the former was significantly higher than disease control group. Conclusion High negative stimulant intensity, unsuitable coping style and deficiency of social support were important psychosocial factors that affecting post-myocardial infarction depression in elderly.
