摘    要:目的:检测有或无症状颈动脉粥样硬化患外周血粘附分子的水平。方法:流式细胞术和酶联免疫吸附法测定42例有症状颈动脉粥样硬化(SCAS)、38例无症状颈动脉粥样硬化(ASCAS)和21例对照组患外周血淋巴和单核细胞CD18,CD50,CD54,(2D49d,CD29,CD62L的表达及血清中可溶性ICAM—1,VCAM—1,P—选择素的浓度。结果:与对照组相比,SCAS和ASCAS组单核细胞CD54阳性百分率及可溶性VCAM-1,P—选择素浓度升高;SCAS组单核细胞CD49d阳性百分率及可溶性ICAM—1浓度升高.SCAS和ASCAS组可溶性ICAM—1,VCAM—1浓度均与颈动脉粥样硬化程度有关。SCAS组可溶性VCAM—1,P—选择素浓度高于ASCAS组。LogistiC多元回归分析证实可溶性VCAM—1是颈动脉粥样硬化出现症状的独立危险因素。结论:粘附分子可以反映颈动脉粥样硬化的程度,VCAM—1在颈动脉粥样硬化患发生缺血性脑血管病中起一定作用。

Abstract:AIM: To assay the level of adhesion molecule of peripheral blood in patients with symptomatic or asymptomatic carotid atherosclerosis. METHODS: By flow cytometry and enzyme lined immunosorbent assay, the expression of CD18, CD50, CD54, CD49d, CD29 and CD62L on lymphocytes and monocytes and the concentration of serum soluble ICAM 1, VCAM 1 and P selectin were measured in 42 patients with symptomatic carotid atherosclerosis (SCAS), 38 patients with asymptomatic carotid atherosclerosis (ASCAS) and 21 control patients. RESULTS: Compared with those of the controls, the positive percentage of CD54 on monocytes and the concentration of soluble VCAM 1 and P selectin were elevated in SCAS and ASCAS groups and the positive percentage of CD49d on monocytes and the concentrations of soluble ICAM 1 were elevated in SCAS group. The concentration of soluble ICAM 1 and VCAM 1 was significantly correlated with the degree of carotid atherosclerosis in SCAS and ASCAS groups. The concentration of soluble VCAM 1and P selectin in SCAS group was higher than that in ASCAS group and the logistic regression analysis revealed that soluble VCAM 1 was an independent risk factor responsible for some symptoms of carotid atherosclerosis. CONCLUSION: Adhesion molecules can reflect the degree of carotid atherosclerosis. VCAM 1 plays a role in carotid atherosclerosis related ischemic cerebral vascular diseases.
