伴有软膜静脉引流的硬脑膜动静脉瘘影像学表现

目的 探讨伴有软膜静脉引流的硬脑膜动静脉瘘(DAVF)患者的CT、MRI表现。方法 回顾性总结了自1990年至今治疗的所有伴有软膜静脉引流的硬脑膜动静脉瘘，分析其CT或MRI表现。结果 自1990年共诊治伴有软膜静脉引流的硬脑膜动静脉瘘35例，有CT、MRI等影像学资料的23例。结论 当平扫CT显示有不明原因的脑水肿，尤其有散在类圆形略高密度异常影像，强化CT显示条形、纡曲的强化影像，则提示患DAVF的可能。在MRI上，头皮及皮层血管的增多、扩张，而没有明显集中的畸形血管团，提示可能患有伴有皮层静脉引流的硬脑膜动静脉瘘。

Imaging of Dural Arteriovenous Fistula Drained by Cortical Veins

Objective To discuss the CT and MRI manifestation of dural arteriovenous fistula (DAVF) with cortical veins drainage.Methods CT and MRI of DAVF patients drained by cortical veins after 1990 were analyzed,and were compared with the cerebral angiography.Results Thirty cases suffering from DAVF drained by cortical veins were treated from 1990.Among them,CT and/or MRI features of 23 cases were analyzed.Conclusion When the cerebral edema,especially diffuse circular hyperintense abnormalities were found on nonenhanced CT,and when the linear tortuose dense enhancement were found on enhanced CT,dural arteriovenous fistula was strongly suggested and possibly drained by cortical veins.Concerning MRI,when the vessels of scalp and cortex increase,and become tortouse and enlarged without the compacted nidus,the possibility of dural arteriovenous fistula with cortical drainage were suggested.
