电视胸腔镜手术23例分析

目的:探讨电视胸腔镜或辅助小切口胸部外科手术的临床应用范围与效果。方法:2005年8月至2006年9月用电视胸腔镜手术(VATS)治疗胸部疾病23例,其中包括自发性气胸、肺大疱切除术14例;凝固性血胸清除、粘连带止血2例;纵隔肿瘤切除术3例;肺叶切除术1例;肺楔形切除术2例;先天性动脉导管未闭(PDA)结扎术1例。结果:治疗效果满意,无手术死亡和并发症发生。结论:VATS具有创伤小、痛苦轻、恢复快和对美容影响小的优点。肺大疱切除可列为首选治疗术式。VATS辅助胸壁小切口是较理想的微创手术方式。目前VATS临床应用有局限性,不能代替常规开胸手术,对手术操作难度大的病变或较大的肿瘤仍选择常规开胸手术为宜。

Video-assisted thoracoscopic surgery in thoracic diseases: A report of 23 cases

Objective:To investigate the application and results of minimal invasive operation with video-assisted thoracic surgery.Methods:23 cases undergone successful video-assisted thoracoscopic surgery((VATS) were analyzed.The data were gathered from August 2005 to September 2006,among which included 14 cases of spontaneous pneumothorax or bunamiodyl excision;2,coagulable haematothorax clear operation or hemostasis of adhering site;3,excision of mediastinal tumor;1,pulmonary lobectomy;2,wedge excision of lung and 1 with ligature of congenital patent ductus artereriosus(PDA).Results:The results were satisfactory and no operative mortality and postoperative complications occurred.Conclusion:The VATS can result in mini-wound,less pain,quick recovery and less damage to the body.It can be used as the first choice for bullae of lung.Combined with chest micro-incision,the VATS has been regarded as an ideal operative approach.However,the VATS can not fill the place of thoracotomy for its limitedness.Regular thoracotomy is preferable in cases of high risks or with larger tumor.
