肛门腺腺癌临床病理观察

目的 探讨肛门腺腺癌的临床病理学特征及诊断、鉴别诊断。方法1例肛门腺腺癌进行光镜、组化和免疫组化观察并结合文献分析。结果 肿瘤由浸润在肛门直肠区肠壁内的小腺管组成，这些腺管杂乱分布，大多分化较好，腔内没有明显的黏液产物。免疫组化标记CEA、CK7、Ki-67和p53（＋），CK20、CDX2、GCDFP15、p63、PSA和mammoglobin均为（-）。VG染色示肿瘤细胞周围有纤维组织反应增生。结论 肛门腺腺癌是一种罕见的肿瘤，易误诊为良性疾病，其诊断和鉴别诊断依赖于病理组织学和免疫组化并需结合临床特点综合分析。

Adenocarcinoma of the anal glands: a clinicopathological observation

Objective To investigate the clinicopatholgic features, diagnosis and differential diagnosis of adenocarcinoma of the anal glands. Methods One ease of adenocarcinoma of the anal glands was analyzed with light microscopic, histochemieal and immunohistochemical analyses and related literature was reviewed. Results The tumor was composed of haphazardly dispersed, small glands with scant mucin production and well differentiation in majority, which invaded the wall of the anorectal area with no obvious intraluminal component observed microscopically. Immunohistochemically, the tumor cells were positive for CEA, CKT, Ki67 and P53, but negative for CK20, CDX2, GCDFP15, P63, PSA and manunoglobin. The fibrous tissues aroud the tumor cells were positive for VG. Conclusion Adenocarcinoma of the anal glands is a rare neoplasm and is easily misdiagnosed as benign disease. The diagnosis and differential diagnosis rely mainly on the general analysis of histopathologic changes, immunohistochemical staining and clinical features.
