热毒宁联合阿奇霉素对支原体感染患儿血清炎性因子及心肌酶水平的影响

摘    要:目的 探讨热毒宁联合阿奇霉素对支原体感染患儿血清炎性因子及心肌酶水平的影响.方法 收集2013年5月至2016年5月我院收治的85例肺炎支原体感染患儿,随机分为对照组43例和观察组42例.两组患儿均给予常规治疗,对照组给予患儿阿奇霉素治疗,观察组给予患儿热毒宁联合阿奇霉素治疗.观察比较治疗前后两组患儿的血清炎性因子,包括白细胞介素-2、-6、-8(IL-2、6、8)、肿瘤坏死因子-α(TNF-α)以及C反应蛋白(CRP),以及心肌酶指标,包括乳酸脱氢酶(LDH)、天冬氨酸氨基转移酶(AST)、肌酸激酶同工酶(CK-MB)以及肌酸激酶(CK)的水平变化,并比较两组临床疗效和不良反应发生情况.结果 治疗后,观察组的临床总有效率为95.2%,显著高于对照组的81.4%(P<0.05).两组的炎性因子水平均较治疗前有显著改善(P<0.05),且观察组的IL-2、TNF-α及CRP的改善幅度大于对照组(P<0.05),而IL-6、IL-8组间比较无明显差异(P>0.05).两组LDH、AST、CK-MB及CK水平较治疗前均明显降低(P<0.05),且观察组的LDH、CK-MB及CK显著低于对照组(P<0.05),但AST水平组间比较差异无统计学意义(P>0.05).此外,两组不良反应发生率比较差异无统计学意义(P>0.05).结论 相较于阿奇霉素单独用药,热毒宁联合阿奇霉素治疗肺炎支原体感染疗效显著,可有效抑制患儿的血清炎性因子和心肌酶水平,且安全性好.

Abstract:Objective To explore the effect of reduning combined with azithromycin on the levels of serum inflammatory factor and myocardial enzyme for children with mycoplasma pneumoniae infection.Methods 85 children with mycoplasma pneumoniae infection were enrolled in our hospital from May 2013 to May 2016, of which patients divided into two groups randomly.All of the children were cured with conventional therapy, and the patients in control group (n=43) accepted azithromycin treatment, and study group (n=42) adopted reduning combined with azithromycin treatment.The serum inflammatory factors including lnterleukin-2,-6,-8 (IL-2, 6, 8), tumor necrosis factor-ɑ (TNF-α) and C reactive protein (CRP), while myocardial enzyme index, including lactate dehydrogenase (LDH), aspartate aminotransferase (AST), creatine kinase isoenzyme (CK-MB) and creatine kinase (CK) were detected respectively and analyzed.The curative effect and adverse reactions were compared and recorded.Results After treatment, the total curative effect of study group was 95.2%, which was significantly higher than those children in control group (81.4%, P<0.05).The level of inflammatory factors were significantly improved compared with before treatment (P<0.05), and the change degree of IL-2, TNF-ɑ and CPR in study group was higher than those children in control group (P<0.05).However, the difference of IL-6 and IL-8 between two groups was not significant (P>0.05).The LDH, AST, CK-MB and CK levels of all children were lower than before treatment (P<0.05), and the LDH, CK-MB and CK levels of children in study group were significantly lower than those in control group (P<0.05).But there was no significant difference of AST cross groups (P>0.05).The difference of adverse reactions between two group was not significant (P>0.05).Conclusion The curative effect of reduning combined with azithromycin for children with mycoplasma pneumoniae infection was better than single azithromycin treatment, which not only inhibited the level of inflammatory factor and myocardial enzyme, but also showed higher level of safety.
