联合检测血清GP73、AFP及AFP-L3在原发性肝癌中的诊断价值

目的 探讨血清GP73、AFP及AFP-L3联合检测在诊断原发性肝癌中的临床价值.方法 选取2013年6月至2015年6月我院肝病中心收治的120例原发性肝癌患者(PHC组),慢性乙肝患者59例(CHB组),肝硬化患者68例(LC组),同时选取同期进行体检的健康人90例作为对照组,比较4组受试者血清GP73、AFP及AFP-L3水平及相关诊断学指标.结果 PHC组、CHB组、LC组患者的GP73、AFP及AFP-L3水平均显著高于对照组,差异有统计学意义(P＜0.05).且PHC组患者的GP73、AFP及AFP-13水平均显著高于CHB组和LC组患者,差异有统计学意义(P＜0.05).对于PHC的诊断,血清GP73的灵敏度显著高于AFP和AFP-L3,差异有统计学意义(P＜0.05).血清GP73、AFP和AFP-L3三项联合检测灵敏度显著高于任一单项指标检测结果,差异有统计学意义(P＜0.05).结论 联合检测血清GP73、AFP和AFP-L3三项指标是诊断原发性肝癌的灵敏指标,能有效提高诊断准确率,具有较高的临床价值.

The Diagnostic Value of Combined Detection of Serum GP73, AFP and AFP-L3 in Primary Liver Cancer

Objective To explore the clinical value of combined detection of serum GP73,AFP and AFP-L3 in primary liver cancer.Methods From Jun 2013 to Jun 2015,120 primary liver cancer (PHC) patients were selected in our liver disease center of our hospital,59 patients with chronic hepatitis B (CHB),68 with liver cirrhosis (LC).At the same time,90 healthy people were assigned as the control group.The serum levels of GP73,AFP and AFP-L3 were compared among the four groups.Results The GP73,AFP and AFP-L3 levels of PHC group,CHB group,LC group were significantly higher than the normal group (P ＜ 0.05),The GP73,AFP and AFP-L3 levels of PHC group were significantly higher than the CHB group and LC group (P ＜ 0.05).For the diagnosis of PHC,the sensitivity of serum GP73 was significantly higher than that of AFP and AFP-L3 (P ＜ 0.05).Sensitivity of serum GP73,AFP and AFP-L3 three joint detection was significantly higher than any of the single indicator (P ＜ 0.05).Conclusion Combined detection of serum GP73,AFP and AFP-L3 is more sensitive and more accurate in diagnosis of primary liver cancer.
