绝经前后亚临床甲减患者骨密度变化的临床分析

摘    要:目的 探讨绝经前后女性亚临床甲状腺功能减退症(亚临床甲减)患者骨密度(BMD)的变化.方法 选择我科就诊亚临床甲减女性患者110例,分为绝经前和绝经后两组,按亚临床甲减病程长短又分为短病程和长病程两个亚组,记录各组年龄、体重指数(BMI)、促甲状腺激素(TSH)、血钙、血磷、绝经年限及BMD.结果 在绝经前、后亚临床甲减患者中,亚临床甲减病程长者腰椎L2-4、左股骨颈及股骨大转子BMD明显低于病程短者(P<0.05).Spearman相关性分析结果表明,BMD与年龄、BMI、TSH、绝经年限、亚临床甲减病程呈明显负相关.Logistic回归分析显示TSH、亚临床甲减病程、绝经年限是骨质疏松的独立危险因素.结论 亚临床甲减病程的长短及绝经与否是女性亚临床甲减患者骨密度的重要影响因素.

Abstract:Objective To explore the effects of bone mineral density(BMD) in female patients with subclinical hypothyroidism(SCH).Methods One hundred and ten females with SCH were chosen from hospitalized patients in our endocrinology.Patients were divided into two groups(premenopausal and postmenopausa1).Each group was then divided into two subgroups(short-term and long-term) based on SCH course.The age,BMI,TSH,blood calcium,blood phosphorus,menopause and BMD were analyzed.Results BMDs of lumbar vertebrae 2-4,left thighbone neck and thighbone big rotor in long-course group were significantly lower than those of short-course group(P <0.05).Spearman correlation analysis results show that BMD and age,BMI,TSH,menopause and SCH was significantly negative correlated (P < 0.05).Logistic regression analysis showed that TSH,SCH and menopause are independent risk factors for osteoporosis.Conclusion The chronic hypothyroidism and menopause are important influence factors in female patients of SCH with low bone density.
