腺苷超声心动图负荷试验对冠心病的诊断价值(附56例报告)

摘    要:目的应用腺苷进行超声心动图负荷试验,探讨其在冠心病临床诊断中的应用价值。方法选择无禁忌证的、临床诊断可疑冠心病的病人56例,冠脉造影前一周内进行腺苷超声心动图负荷试验,剂量为140μg·kg^-1·min^-1,输注速率(ml/min)=0.14mg·kg^-1·min^-1×体重(妇)/腺苷浓度(3mg/ml)。观察不同负荷状态室壁收缩运动情况,判断室壁运动异常的部位与范围,评价该试验对冠心病诊断的敏感性、特异性等指标。结果腺苷超声心动图负荷试验诊断冠心病的敏感性、特异性、准确性分别为85.3%、71.4%和80.0%。腺苷负荷后副反应轻微,停药后很快消失,病人多能耐受,无严重不良反应发生。结论腺苷超声心动图负荷试验是诊断冠心病安全、方便、准确的一种新方法。

Abstract:Objective To evaluate the diagnostic value of adenosine stress echocardiography in patients with coronary heart disease (CHD). Metheds Adenosine stress echocardiography was performed in 56 patients with suspected CHD before coronary angiography within one week. The dose was 140 μg·kg^-1·min^-1, the dripping rate was 0. 14 mg·kg^-1·min^-1 )〈 Weight (kg) / Adenosine concentration (3 mg/ml). The left ventricle was divided into 16 segments and the wall motion and contraction were observed for judging the location and regions of segments with RWMA. Results The diagnostic sensitivity, specificity, and accuracy of CHD by adenosine stress echocardiography was 85.3%, 71.4% and 80.0%, respectively. Side effects due to adenosine infusion were always minimal and well tolerated by the patients and disappeared within seconds after termination of infusion. Conclusion Adenosine stress echocardiography was a safe, convenient and accurate method to diagnose CHD.
