我国城市及县域医疗联合体绩效评估研究

摘    要:目的从资源下沉、双向转诊和辐射带动3个维度,对2018—2019年我国城市医疗联合体与县域医疗联合体建设的效率效益进行评价,并对比分析两者之间的优势与劣势,为进一步推进医疗联合体建设提供参考。方法采用TOP-SIS法、秩和比法及二者模糊联合法进行评价。结果我国医疗联合体建设有所成效,县域医疗联合体效率效益低于城市医疗联合体,但增长幅度大于城市医疗联合体,区域之间效率效益差异较大,北部普遍低于南部。结论城市医疗联合体的优点在于支援力度大、技术水平高、辐射范围广,县域医疗联合体的优点在于县域范围小、稳定性高、更利于内部管控和资源整合,二者应相互结合、共同改进。

Research of Performance Evaluation of Urban and County Medical Alliance in China

Abstract:Objective From the three dimensions of resource sinking,two-way referral and radiation drive,to evaluate the efficiency and effectiveness of the development of China’s urban and county medical alliance from 2018 to 2019,and to compare and analyze the advantages and disadvantages between the two,in order to provide reference for promoting the construction of medical alliance.Methods Using TOPSIS method,RSR method and the fuzzy combination method of the two.Results The construction of China’s medical alliance has been effective,the efficiency and effectiveness of the county medical alliance is lower than that of the urban medical alliance,but the growth is greater than that of the urban medical alliance.The efficiency and benefits vary greatly between regions,and the northern region is generally lower than that in the southern region.Conclusion The advantages of the urban medical alliance are its strong support,high technical level and wide radiation range,and the advantages of the county medical alliance are that the county is small,stable,and more conducive to internal control and resource integration.The two should be combined and improved together.
