利水软肝汤、中药敷脐联合温针灸对肝硬化腹水患者肝功能、尿量和门静脉血流动力学的影响

目的:观察利水软肝汤、中药敷脐联合温针灸对乙型肝炎肝硬化腹水患者肝功能、尿量和门静脉系统血流动力学的影响。方法:选取2016年1月至2019年12月104例乙型肝炎肝硬化腹水患者进行研究,随机分为观察组和对照组,每组52例。对照组患者采用西医常规治疗。观察组患者在对照组治疗的基础上采用利水软肝汤、中药敷脐联合温针灸治疗。比较两组患者治疗后的总有效率、治疗前后腹围、体重、肝功能、24 h尿量、腹水暗区深度、门静脉系统血液动力学的变化。结果:治疗1个月后,观察组患者总有效率高于对照组,差异有统计学意义(P<0.05);观察组患者肝功能和门静脉系统血液动力学改善程度优于对照组,差异有统计学意义(P<0.05)。观察组患者腹围、体重和腹水暗区深度低于对照组;观察组24 h尿量高于对照组,差异有统计学意义(P<0.05)。结论:利水软肝汤、中药敷脐联合温针灸治疗乙型肝炎肝硬化腹水患者的疗效较好,能明显改善患者肝功能,降低腹水量、增加24 h尿量,进而降低腹围、体重,明显改善门静脉系统的血流动力学。

Effects of Lishui Ruangan decoction,umbilical application of traditional Chinese medicine combined with warm acupuncture on liver function,urine volume and portal hemodynamics in patients with hepatitis B and ascites

Objective:To observe the effects of Lishui Ruangan decoction,Chinese medicine umbilical application combined with warm acupuncture on liver function,urine volume,and portal hemodynamics of hepatitis B liver ascites.Methods:One hundred and four patients with hepatitis B cirrhosis ascites from January 2016 to December 2019 were selected and randomly divided into two groups,52 cases in the observation group and 52 cases in the control group.The control group was treated with conventional western medicine.The observation group was treated with Lishui Ruangan decoction,umbilical application of traditional Chinese medicine,and warm acupuncture based on conventional western medical treatment.The total effective rate,liver function,abdominal circumference,body weight,24 h urine volume,ascites dark area depth,and portal hemodynamics were compared between the two groups after treatment.Results:After one month of treatment,the total effective rate of the observation group was higher than that of the control group,the difference was significant(P<0.05),the improvement of liver function and portal hemodynamics in the observation group was better than that in the control group,the difference was significant(P<0.05).The abdominal circumference,body weight,and ascites dark area depth of the observation group were lower than those of the control group,and the 24 h urine volume of the observation group was higher than that of the control group,the difference was significant(P<0.05).Conclusion:Lishui Ruangan decoction,navel application of Chinese medicine combined with warm acupuncture in the treatment of hepatitis B cirrhosis ascites patients had a better curative effect,significantly improved liver function,reduced ascites volume,increased 24 h urine volume,and then reduced abdominal circumference and body weight,and significantly improved the hemodynamics of portal vein system.
