Granulocyte Expression and its Correlation with Chlorpromazine Equivalent in First Episode Mental Illness and Schizophrenia

[Objective]To investigate the correlation between the changes of granuloeyte level and chlor-promazine equivalent in patients with first episode mental llness and schizophrenia.[Methods]Prospective se-lection of patients with first episode psychosis and schizophrenia admitted to our hospital from May 2017 to February 2020 as the research objeet,and the same period of adult healthy volunteers as the conirol group(n=62).The levels of neutrophils at admitment(T0)and 6 weeks after therapy(T6)were recorded.and the positive and negative symptom scale(PANSS)was used to evaluate the mental symptoms.The cumulative chlorpromazine equivalent was calculated according to the dose conversion standard,and the correlation be-tween granulocyte level and clinical symptoms,chlorpromazine equivalent was analyzed.[Results]At T0,compared with the control group,the levels of neutrophils and CRP in patients with mental ilness(FEP group+SCHIZ group)were increased,while the levels of cosinophils were decreased,and the differences were sta-tstically significant(P<0.05).At T6,BMI,neutrophils.eosinophils,monocytes and CRP levels in patients with mental ilness(FEP+SCHIZ group)were higher than those in the control group(P<0.05).Com-pared with T0,at T6,the BMI and eosinophil levels of the two groups were increased.and the PANSS score.neutrophil.monoeyte and CRP levels were decreased,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that PANSS score of patients with mental ilness(FEP+schiz group)was positively correlated with neutrophil and serum CRP level(P<0.05).and negatively correlated with eo-sinophil level(P<0.05);serum CRP level of patients with FEP was positively correlated with PANSS score(P<0.05).Pearson correlation analysis also showed that chlorpromazine equivalent was negatively correlated with neutrophil level in FEP patients(r=-0.284.P=0.001),positively correlated with eosinophils(r=0.211,0.011),and no other correlation(P>0.05).[Conclusion]The occurrence of mental ilness and schizo-phrenia may be related to the changes of innate immune system.A certain amount of chlorpromazine can im-prove the clinical sympioms of FEP and SCHIZ patients,and monitoring the levels of neutrophils,eosinophils and serum CRP can provide reference for judging the degree of the disease.
