电视胸腔镜手术在胸部疾病治疗中的应用(附305例报告)

目的总结胸部疾病电视胸腔镜手术（VATS）经验并探讨电视胸腔镜手术治疗方法、原则和发展方向。方法对我院1999年6月至2004年6月采用VATS治疗的305例胸部疾病进行回顾性分析。手术均在静吸复合全麻双腔气管插管下进行。结果305例患者均痊愈出院．围手术期死亡率为0．并发症（血胸3例）发生率为0．98％（3／305）。结论VATS具有创伤小、痛苦轻、恢复快和对美容影响小的优点．可列为自发性气胸、血气胸、肺大疱的首选治疗术式。目前VATS临床应用有局限性，尚不能完全代替常规开胸手术，应严格掌握VATS适应证。

Video-assisted Thoracic Surgery in the Treatment of Thoracic Diseases: A Retrospective Study of 305 Cases

Objective To summarize the experience of video-assisted thoracic surgery(VATS) in the treatment of thoracic diseases and discuss the role of VATS in the diagnosis and therapy of thoracic diseases.Methods From June 1999 to June 2004,clinical data were reviewed from 305 patients successfully undergoing VATS in the thoracic department of our hospital.General anesthesia and double cavity intubation was used during operation in all patients.Results 305 patient underwent VATS procedures.The overall incidence of perioperative complications(hemopneumothorax: 3 cases) was 0.98%(3/305),and no patient died during the operation.Conclusion VATS is a safe,effective,less traumatic and cosmetically excellent procedure in the selected patients with thoracic diseases.It can be used as the first choice for spontaneous pneumothorax or hemopneumothorax and bulla of lung.At the same time,it was considered that VATS can not replace thoracotomy at present.
