亚低温联合异丙酚对高血压脑出血术后患者疗效分析

摘    要:目的探讨亚低温联合异丙酚对高血压脑出血手术后患者的疗效。方法选择高血压脑出血手术后患者76例,随机分为常规治疗组(对照组)38例、亚低温联合异丙酚治疗组(治疗组)38例。监测两组患者术后恢复过程中的平均动脉压(MAP)、心率(HR)、颅内压(ICP)、血糖、血电解质、烦躁及抽搐情况,以及术后第14天的临床神经功能缺损程度评分,1月GOS评分。结果治疗组与对照组相比,高血压脑出血早期的颅内压、平均动脉压、血糖、体温、心率显著下降(P〈0.05),脱水剂使用量及次数减少(P〈0.05),烦躁、抽搐、肺部感染、电解质紊乱较对照组减少(P〈0.05),术后GCS,GOS评分较对照组显著改善(P〈0.01)。结论亚低温联合异丙酚治疗高血压脑出血术后患者,具有降低颅内压,减少并发症,提高生存质量,无严重并发症。

Abstract:Objective To investigate the effects of mild hypothermia combined with propofol on postoperative hypertensive intracerebral hemorrhage. Methods Seventy- six patients received hypertensive intracerebral hemorrhage surgery were randomly divided into two groups: control( conventional treatment) and experimental groups.The therapeutic effects in two groups during recovery were evaluated via those indexes as follows: mean arterial pressure( MAP),heart rate( HR),intracranial pressure( ICP),blood glucose,blood electrolytes,state of irritability,times of convulsion and score of clinical neural function 14 days after operation and GOS 1 month after surgery. Results Compared with the control group,the mean arterial pressure,blood glucose,body temperature,heart rate,usage and times of dehydrating agent,the chances of irritability,convulsion,pulmonary infection and abnormal electrolyte were decreased and the scores of GCS and GOS were improved in the experimental group.Conclusion Mild hypothermia combined with propofol could lower the intracranial pressure and reduce the stimulated responses and complications on postoperative hypertensive intracerebral hemorrhage and then effectively improve the quality of subsistence.
