二尖瓣及主动脉瓣联合瓣膜置换术围术期处理

目的：探讨二尖瓣及主动脉瓣联合瓣膜置换术围术期的处理。方法：对本院2007年1月～2009年3月收治的50例联合瓣膜病患者进行联合瓣膜置换术,加强围术期的处理,降低手术并发症和病死率。结果：全组50例患者中,除2例患者由于低心排和肾功能、呼吸功能衰竭死亡外,其余患者可见明显的房室缩小,二尖瓣瓣口面积增大,主动脉瓣瓣口峰值跨瓣压差减小,肺动脉收缩压降低,改善了心功能,心功能恢复至Ⅰ级40例,Ⅱ级6例,Ⅲ级2例。结论：加强联合瓣膜置换手术的围术期处理,可有效降低手术风险和病死率。

Perioperative management about mitral and aortic valve replacement combined treatment

Objective：To investigate the perioperative management about mitral and aortic valve replacement combined treatment.Methods：50 cases of valve disease combined joint valve replacement surgery treated during 2007-2009 in our hospital,to strengthen perioperative treatment and reduce surgical complications and mortality.Results：In all 50 patients,except 2 patients due to low cardiac output and renal function,respiratory failure and death,the remaining patients significantly reduced the increase of AV,mitral valve area increased significantly,aortic prosthetic mouth significantly reduced the peak transvalvular pressure,pulmonary artery systolic pressure decreased significantly improved heart function,heart function to 40 cases of grade Ⅰ,Ⅱ grade in 6 cases,Ⅲ grade 2 cases.Conclusion：Strengthening the joint valve replacement surgery perioperative management can reduce surgical risk and mortality.
