BiPAP呼吸支持模式对急性左心功能衰竭的疗效观察

摘    要:目的:探讨经面罩双水平气道正压通气(BiPAP)呼吸支持模式对急性左心功能衰竭的治疗作用。方法:将74例左心功能衰竭患者,随机分为治疗组及对照组,两组患者均给予抗心力衰竭治疗,治疗组患者在抗心力衰竭的基础上给予BiPAP模式呼吸支持,观察两组患者的血流动力学、呼吸功能变化及临床疗效。结果:HR、SBP、SpO2及PaCO2治疗组分别为(94.0±18.2)/min、(126.0±11.4)mmHg、(93.0±8.2)%和(36.0±6.5)%,对照组分别为(109.0±17.6)/min、(104.0±9.5)mmHg、(84.0±9.7)%和(49.0±11.3)%,治疗组患者HR、PaCO2低于对照组,SBP、SpO2高于对照组,临床有效率治疗组为91.9%,对照组为78.4%,治疗组高于对照组。临床有效率亦高于对照组。结论:BiPAP呼吸支持模式能够改善左心功能衰竭患者的血流动力学变化,提高左心功能患者的临床治疗效果。

Curative effect of ventilatory support with BiPAP pattern on acute left heart failure

Abstract:Objective:To investigate the therapeutic efficacy of ventilatory support with Bi-level positive airway pressure(BiPAP) pattern on acute left heart failure.Methods:74 cases of patients with acute left heart failure were randomly divided into therapy group and control group,all patients received treatment with retrieving heart failure and patients in trerapy group reserived treatment with ventilatory support with BiPAP pattern,the difference in ratory function,aemodynamics and clinical therapeutic efficacy were observed.Results:HR,SBP,SpO2 and PaCO2 was(94.0±18.2)/min、(126.0±11.4)mmHg,(93.0±8.2)% and(36.0±6.5)% in therapy group,(109.0±17.6)/min,(104.0±9.5)mmHg,(84.0±9.7)% and(49.0±11.3)% in control group,in comparison with patients in control group,the therapy group's HR,PaCO2 was lower than the control group and SBP and SpO2 was higher than control group,clinical effective power was 91.9% in therapy group and was 78.4% in control group,clinical effective power was hogher in therapy groups than in therapy group.Conclusion:Ventilatory support with BiPAP pattern on acute left heart failure is helpful for improving aemodynamics and increasing clinical therapeutic efficacy in left heart failure patients.
