中空螺钉钛丝张力带内固定治疗尺骨鹰嘴骨折

目的：探讨中空螺钉钛丝张力带内固定治疗尺骨鹰嘴骨折的疗效。方法：2010年4月～2011年7月共收治14例尺骨鹰嘴骨折患者,男9例,女5例,年龄20～60岁,均为闭合性尺骨鹰嘴骨折,手术采用肘后直切口,复位骨折后用2枚1.5mm的导针经鹰嘴平行钻入,钻孔后沿导针拧入4.5mm半螺纹自攻中空螺钉,骨折远端横钻孔穿入1.3mm钛丝形成＂8＂字张力带结构,术后早期开始功能锻炼。结果：14例患者术后（1～12个月）随诊,随诊6周时所有患者骨折均达到临床愈合标准,于平均3个月达到骨性愈合。随诊期间无螺钉、钛丝松动、断裂、骨折分离、感染及排斥反应等并发症的发生。结论：中空螺钉钛丝张力带内固定具有操作简便,固定牢靠,可以早期活动关节及疗效确切等优点,是一种治疗尺骨鹰嘴骨折的较好方法。

Internal fixation with hollow screw and titanium wire tension band in treatment of ulnar olecranal fracture

Objective：To investigate the therapeutic effects of internal fixation with hollow screw and titanium wire tension band in the treatment of ulnar olecranal fracture.Methods：From April 2010 to July 2011,we treated 14 patients who with olecranon fracture,including 9 males and 5 females,aged 20-60 years old.All the patients had closed ulnar olecranal fracture,receiving posterior cubital straight incision during operation.Two 1.5 mm guide needles were used for parallel drill through the olecranon after reduction of fracture.The 4.5 mm partial thread self-tapping hollow screw was screwed in along the guide needles.The 1.3 mm titanium wire was penetrated through the horizontal drill hole of the fracture distal end,forming an 8-shaped tension band structure.Postoperative function exercises started in the early stage.Results：14 patients received（1-12 months） follow-up.All the patients＇ fractures reached the clinical healing standards in the 6 th week of follow-up and reached osteal healing in an average of 3 months.No screw and titanium looseness,rupture,fracture separation,infection and rejection reaction and other complications occurred.Conclusion：Internal fixation with hollow screw and titanium wire tension band,showing simple operations,stable fixation,early joint activities,effective therapeutic effects and other advantages,is a relatively good treatment method of ulnar olecranal fracture.
