异丙酚复合阿托品用于结肠镜检查的研究

目的比较异丙酚、异丙酚复合阿托品应用于结肠镜检查的安全性、舒适性及实用性。方法选择需结肠镜检查患者60例,随机分为A、B 2组各30例,A组缓慢静注异丙酚(1~2 mg/kg),B组缓慢静注异丙酚(1~2 mg/kg)和阿托品(0.5 mg)混合液,用药至患者无意识、睫毛反射消失后开始检查,后根据手术需要和患者情况追加异丙酚(30~50 mg)至手术结束。连续监测并记录麻醉前、手术开始至手术结束时2组患者的收缩压(SBP)、HR、Sp(O2),术后苏醒时间及异丙酚用量。结果A组术中HR与术前基础值相比有显著性差异(P<0.01);B组麻醉前后无显著性差异(P>0.05)。A、B 2组SBP与术前基础值相比均下降明显,有显著性差异(P<0.01);2组异丙酚用量相比,B组明显少于A组(P<0.01);2组患者对术中不适均无记忆。结论异丙酚能减轻结肠镜检查的痛苦,异丙酚复合阿托品能减轻异丙酚的心血管不良反应,并减少异丙酚的用量。

Study of propofol combined with atropine in colonoscopy

Objective It is to compare the safety, comfortability and practicality of propofol and propofol combined with atropine in colonoscopy. Methods 60 patients needing colonoscopy were divided into group A and group B. Each group had 30 cases. Group A was given propofol 1~2 mg/kg and group B was given propofol 1～2 mg/kg combined with atropine 0.5 mg by intravenous injection slowly. Colonoscopy was done when the patients had no consciousness and lash reflex. Propofol (30～50 mg) was added during the operation according to requirement. SBP, HR, Sp(O_2), analepsia time and propofol dosage were monitored continuously and recorded before, during and after operation. Results Compared with those before colonoscopy, HR in group A decreased significantly after anesthesia (P<0.01), but that in group B had no significant changes (P>0.05). SBP in the both groups decreased obviously during the operation compared with that before anesthesia (P<0.01). The propofol dosage in group A was much less than that in group B (P<0.01). The patients in both groups had no unwell memory of the operation after the colonoscopy was completed. Conclusion Propofol can relieve the distress of colonoscopy. Propofol combined with atropin can relieve the untoward reaction of cardiovascular system and decrease the propofol dosage.
