CDFI 联合 B-flow 评价颅外颈内动脉狭窄程度的准确性探讨

摘    要:目的:探讨彩色多普勒血流成像( CDFI )与实时灰阶二维血流显像( B-flow )联合评价颅外段颈内动脉( EICA )粥样硬化斑块性狭窄程度的准确性。方法颅外颈内动脉粥样硬化斑块性狭窄病例72例,应用CDFI,CDFI联合B-flow两种方法测量同一部位面积狭窄率,将狭窄程度分为4级,每级分别与DSA检查结果相比较。结果72例患者共检出病变血管123条。以DSA为金标准,CDFI联合B-flow对123条不同狭窄程度血管评价的敏感性、特异性较单一CDFI有明显提高,特别是中度以上狭窄,且与DSA有着更高的一致性。结论B-flow能够弥补CDFI的某些不足,进一步提高超声诊断颅外颈内动脉狭窄的准确性。

Abstract:Objective To explore the accuracy of the degree of stenosis of extracranial internal carotid artery ( extracranial internal carotid artery ,EICA) by color Doppler flow imaging ( color Doppler flow imaging ,CDFI) combined with real-time gray-scale 2-D flow imaging(B-flow).Methods Extracranial internal carotid atherosclerotic plaque steno-sis 72cases,application of CDFI ,CDFI combined with B-flow two kinds of methods to measuring the same area stenosis rate,the degree of stenosis were divided into four levels ,each level was compared with DSA respectively .Results One hundred and twenty-three vascular lesions were detected from 72 cases of patients .According to DSA as the gold stand-ard ,CDFI combined with B-flow on 123 different degree of stenosis vascular evaluation of sensitivity ,specificity compared with single CDFI increased ,especially moderate stenosis ,and had a higher consistency with DSA .Conclusion B-flow can make up for some deficiencies of the CDFI , further improve the stenosis accuracy for sonographic diagnosis of ex-tracranial internal carotid artery .
