糖尿病视网膜病变患者血小板平均体积与视网膜厚度及尿清蛋白排泄率的关系

目的探讨糖尿病视网膜病变(DR)患者血小板平均体积(MPV)与视网膜厚度(RT)及尿清蛋白排泄率(UAER)的关系及意义。方法对2004年4月至2006年7月新乡医学院第一附属医院诊治的55例DR患者(DR组)MPV、RT及UAER进行测定,并与30名健康对照组比较。结果DR组MPV、RT及UAER均显著高于健康对照组(P<0.01);DR患者尿清蛋白大量组MPV和RT显著高于尿清蛋白正常组(P<0.01)和尿清蛋白微量组(P<0.01或P<0.05),尿清蛋白微量组显著高于尿清蛋白正常组(P<0.01或P<0.05);RT与MPV及UAER均呈正相关(r=0.547,P<0.05;r=0.823,P<0.01)。结论MPV、RT及UAER的检测,对DR的发生发展及早期诊断和治疗具有重要意义,是反映DR微血管病变程度及病变范围的指标。

Relationship between mean platelet volume and retinal thickness and urinary albumin excretion rate in diabetes mellitus patients with retinopathy

Objective To study the changes of mean platelet volume and urinary albumin excretion rate and retinal thickness and relationship among them in diabetes mellitus patients with retinopathy(DR).Methods Mean platelet volume was measured with MEK-6318K automated blood cell counter;retinal thickness was measured with optical coherence tomography(OCT);urinary albumin excretion rate was measured with rate nephelometry in DR,which were compared with those of 30 normal controls.Results The mean platelet volume and retinal thickness and urinary albumin excretion rate in DR were significantly higher than those in normal controls(P<0.01);the mean platelet volume and retinal thickness in DR with macro-albuminuria were significantly higher than those in DR with normo-albuminuria(P<0.01)and in DR with micro-albuminuria(P<0.01 or P<0.05);the mean platelet volume and retinal thickness in DR with micro-albuminuria were significantly higher than those in DR with normo-albuminuria(P<0.01or P<0.05);both mean platelet volume and urinary albumin excretion rate were positively correlated with retinal thickness.Conclusion The measurement of mean platelet volume and retinal thickness and urinary albumin excretion rate is of clinical value of application and study in the early diagnosis or prevention and treatment of DR.
