摘    要:对76例乳腺针吸细胞涂片,进行了AgNOR分型和计数。其中良性病变12例,恶性病变64例。染色方法及计数按常规进行,分型则参考Meehan法,按自订的三种参数判断标准进行观察。结果:AgNOR计数,良性病变的均值为3.93±0.46;恶性病变均值为9.18±2.12,两者差异非常显著(P<0.001)。计数最适诊断的分界值为4.7。AgNOR分型,良性病变平均积分是3.03±0.46;恶性病变为6.30±0.76,两者相差非常显著(P<0.001)。分型最适诊断的分界值为4.1。此外,对分型的三种参数分别作了积分,结果均具统计学意义(P<0.001)。AgNOR分型是一种简便、快速、可重复的计量方法,技术易掌握,观察者之间重复性高,诊断准确率可高达90%。由于AgNOR分型是一多参数的检测,故结果对诊断有实用价值,如与细胞学检查联用则更有益。

Abstract:76 pieces of smears of the breast fine needle aspiration cell have been classified and counted for AgNOR.There were 12 cases of benign lesions and 64 cases of malignant lesions among them. The staining and Count were done as usual. The classification was analyzed in according to the self-drafting standard of 3-parameter-judgement which referred to Meehan's method. The result shows that as AgNOR count,the average integral value of the benign lesions is 3.93±0.46,and of malignant lesins is 9.18±2.12.the difference bstween the two values is very obvious(P<0.001). The critical value best suitable to diagnosis is 4.7; as AgNOR classifying, the above value is 3.03±0.46 and 6.03±0.76 respectively,the difference between the two values is very obvious, too (P<0.001). The critical value best suitable for diagnosis is 4.1.In addition,three parameters for classification were integrated over respectively and all of the results are of significance in statistics (P<0.001). AgNOR classification is a simple,fast and repeatable method of count because this technique is easy to handle and the repeatability is high in cases and the diagnostic accuracy is up to 90%. AgNOR classifying is a kind of measurement with multiparameter,so its result has practical value,and is more helpful if combined with the che ck of cytology.
