小肾癌的螺旋CT诊断

摘    要:目的:分析小肾癌螺旋CT多期增强扫描表现,探讨其扫描计划.材料和方法: 收集经手术病理证实的小肾癌17个,行螺旋CT平扫和皮质期、实质期及肾盂期增强扫描,分析其表现.结果:17个癌灶中,平扫11个等密度,稍高密度及低密度各3个,2个伴斑点样钙化.增强扫描皮质期7个癌灶与肾皮质增强程度相近,10个癌灶皮质期增强程度低于肾皮质,14个癌灶邻近肾皮质显影模糊或中断;实质期,肾盂期癌灶增强程度下降,肾盂期癌灶境界显示清晰,15个癌灶邻近肾盏或肾盂有受压征象.结论:螺旋CT多期扫描对小肾癌是非常有价值的诊断方法,扫描计划应包括平扫、皮质期及肾盂期增强扫描.

Abstract:Purpose: To analyze the multi phasic helical CT appearances and investigate the protocal of small renal cell carcinoma(SRCC). Materials and Methods: The helical CT features of 17 SRCCs in 16 patients proved by operation and pathology were studied retrospectively. Results: CT showed isodense mass in 11 SRCCs, but hypo and slightly hyperdense mass respectively in 3 SRCCs, with punctural calcification in 2 masses. After administration of contrast media,in the cortical phase the tumor was enhanced,with the same degree to the cortex in 7 SRCCs,and lower degree in 10 SRCCs. The cortex adjacent the tumor was hazy or broken in 14 SRCCs. In paranchymal or pyeographic phase,the degree of the tumoral enhancement became gradually lower,but the tumoral margin was more clear. The renal calices and pelvis near the tumor were compressed in 15 SRCCs. Conclusion: Multi phasic helical CT is a very valuable method in diagnosis of SRCCs, and the protocal must include plain scan, cortical and pyelographic phases in enhanced scan.
