脑皮质发育不良所致癫痫的临床特点分析

目的:探讨脑皮质发育不良(MCD)所致癫痫的临床特点。方法:对28例2004年1月～2010年12月在我院神经内科经头颅MRI诊断为脑皮质发育不良所致癫痫患者的临床资料进行回顾性病例对照研究。结果:与对照组比较,28例皮质发育不良所致癫痫患者的年龄为(12.29±5.65)岁;复杂部分性发作继发全面性强直-阵挛性发作(GTCS)占57.1%(16例);伴认知功能障碍占28.6%(8例);发作间期脑电图异常占53.6%(15例),其中25%(7例)有节律性放电;78.6%(22例)对3种或以上抗癫痫药物疗效不佳。结论:脑皮质发育不良所致癫痫患者的发病年龄在儿童期多见,发作类型以复杂部分性发作继发GTCS为主,部分合并认知功能障碍,发作间期异常脑电检出率较高,大部分患者为药物难治性癫痫。

Clinical features of seizure caused by malformations of cortical development

Objective： To investigate the clinical features of seizure caused by the malformations of cortical development（MCD）.Methods： Clinical data of 28 patients who were diagnosed as seizure caused by the MCD in the Department of Neurology in our hospital from January 2004 to December 2010,were retrospectively analyzed.Results： Compared with control group,the age of the 28 patients of epilepsy due to MCD was（12.29±5.65）yeas old.Complex partial seizures secondary generalized tonic clonic seizure（GTCS） accounted for 57.1%（16 cases）.Associated with cognitive dysfunction accounted for 28.6%（8 cases）.In the interictal period with EEG abnormalities accounted for 53.6%（15 cases）,including 7 cases（25%） with rhythmic discharge.And poor efficacy of three or more antiepileptic drugs accounted for 78.6%（22 cases）.Conclusions： Epilepsy due to MCD is common in children.Complex partial seizures secondary GTCS is a major seizure types.Some patients are coplicated with cognitive dysfunction,with higher incidence of abnormal EEG in interictal period.And most patients have a variety of antiepileptic drug resistance.
