涤纶片和牛心包预防兔肺减容术后肺切缘漏气

摘    要:目的:研究涤纶织片加固钉夹缝合器行兔肺减容手术(LVRS)减少肺切缘漏气时间的效果。方法:新西兰大白兔30只,采用木瓜蛋白酶雾化吸入诱发肺气肿模型后随机分为3组,每组10只:A组为涤纶片处理组;B组为牛心包处理组;C组为无加固材料对照组。分别行肺减容手术,术后检测持续漏气时间及作病理组织学检查。结果:应用涤纶片与牛心包加固的钉夹缝合器,两组间术后持续漏气时间无明显差异(P>0.05),而与对照组比较有明显差异(P<0.01);牛心包肺切缘可见有大量的单核、多核细胞聚集在牛心包接触面,伴有移植胶原的降解,涤纶片肺切缘周围仅有少量非特异生淋巴细胞浸润;木瓜蛋白酶诱发的肺气肿模型在形态学上完全符合肺气肿的病理学特征。结论:涤纶织片加固钉夹缝合器行肺减容手术可减少持续漏气时间。

Abstract:AIM To study curative effect of Dacron strips on staple lines to prevent air leaks in lung volume reduction operations in rabbits. METHODS The emphysema model was induced by inhaling atomized papain in 30 male New Zealand White rabbits which were randomly divided into 3 groups: Dacron group (A) ( n =10), bovine pericardium group (B)( n =10) and no buttressed material control (C) ( n =10 ). The lung volume reduction operations were performed. The durations of air leak were documented and slides of lung tissue were examined under light microscope. RESULTS There was no significant difference in the durations of air leak between the bovine pericardium group and the Dacron group ( P >0.05). However there was a significant difference in another group with no buttered material ( P <0.01). Histologic study revealed a dense mononuclear and multinuclear giant cell infiltration at the interface between bovine pericardium and surrounding the host tissue, with focal degradation of implant collagen. Dacron only elicited a little nonspecific lymphocytic infiltration. The emphysema model induced by papain accorded with the pathological characteristic of pulmonary emphysema. CONCLUSION Dacron sleeves for buttressing staple lines are effective in lung volume reduction operation.
