外周动脉内膜-中层厚度与冠状动脉粥样硬化的相关性

摘    要:目的研究外周动脉(颈总动脉、颈动脉窦和股动脉)内膜-中层厚度(IMT)与冠心病患者冠脉病变的相关性。方法将300例拟诊为“冠心病”的住院患者行冠状动脉造影,并对所有研究对象进行外周动脉IMT和斑块的超声检测,而后评估外周动脉IMT与冠心病患者冠脉病变程度的相关性。结果在冠心病组外周动脉IMT值明显高于正常对照组(颈动脉0.94 vs 0.58毫米,颈动脉窦1.27 vs 0.65,股动脉1.47 vs 0.70,P值均〈0.01)。而且三支冠脉病变组IMT值高于单支组和双支组(P〈0.05)。结论外周动脉IMT值可作为早期动脉粥样硬化的标志并且可能反映冠状动脉粥样硬化病变的严重性。

Abstract:Objective:The aim of the study was the assessment of intima - media thickness (IMT) in peripheral arteries ( the carotid and the femoral artery) and its correlation with the extent of coronary artery disease (CAD). Methods: 300 patients who had undergone coronary angiography due to symptoms of CAD were enrolled. The ultrasound measurement of IMT in the common carotid artery (CCA), carotid bulb and common femoral artery (CFA) was performed. The relationship between IMT and the extent of CAD was evaluated. Results:Signi cantly higher values of IMT in the peripheral arteries were observed in patients with CAD than in those without ( CCA -0. 94 v. s. 0.58 mm, carotid bulb - 1.27 v. s 0.65, CFA - 1.47 v. s 0.70 respectively, P 〈 0.01 ). And higher values of IMT in patients with three - vessel disease were observed than in those patients with one or two - vessel disease ( P 〈 0.05 ). In multifactoral analysis we found that coronary artery atherosclerosis is strongly influenced by age, BMI, and value of IMT in the femoral artery (P 〈 0.01 ). Conclusion: IMT in peripheral arteries may be used as an early marker of atherosclerosis and can re? ect the severi .ty of CAD.
