住院精神病人保护性约束调查及护理对策

目的探讨精神科临床应用保护性约束的原因,对病人的身心影响及护理对策。方法采用自行设计的调查问卷,对入住我院封闭病区经历过保护性约束的精神病人进行调查分析。结果81例约束病人中,约束的主要原因是攻击行为、行为紊乱占70.3%;约束持续时间超过1天者占50.6%;约束后出现不良心理反应者占89%;躯体不良影响者占33.4%。结论保护性约束是重症精神病人防范危害行为的有效措施,但合理使用可降低对病人的负面影响。

A survey on application of protective constraint among psychotic inpatients and corresponding nursing strategy

Objective To explore the reasons ,duration ,frequency ,psychological influence of protective constraint applied to psychotic inpatients and corresponding nursing strategy.Methods A total of 81 cases of patients accepted protective constraint in the ward in close-ended management enrolled in this study. The reasons, duration and the psychological influence of the protective constraint applied to these psychotic inpatients were evaluated by nurses using the self-designed questionnaire.Results As for the results of the investigation, aggressive behavior and conduct disorder were the main reasons of constraint, which occupied 37% and 33.3% respectively. The duration of 50.6% patients were more than 24 hours. Accordingly, the harmful influence of constraint to physical and mental health were obvious, 89% and 33.4% of the 81 cases of patients were reported experiencing negative mental and physical responses respectively.Conclusion It is suggested that the application of protective constraint to psychiatric patients in the ward in close-ended management of psychiatry hospital is effective and necessary for safety management. But the harmful influence of constraint should be considered.
